PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK
In consideration of the services of CVCTC/CVP owners, agents, officers, employees and all other persons or
entities action in any capacity on their behalf (hereinafter collectively referred to as CVCTC/CVP, I hereby
agree to release, discharge, and hold harmless CVCTC/CVP on behalf of myself, my children, my parents, my
heirs, assigns, personal representative and estate as follows:
1.
2.
3.
4.
5.
6.
7.
I understand and acknowledge that the activities that I or my child engage in while on the premises or under the
auspices of CVCTC/CVP pose known and unknown risks which could result in injury, paralysis, death, emotional
distress, or damage to me, my child, to property, or to third parties. The following describes some, but not all of
those risks: Cheerleading and gymnastics, including performances of stunts and use of trampolines, entail certain
risks that simply cannot be eliminated without jeopardizing the essential qualities of the activity. Without a
certain degree of risk, cheerleading students would not improve their skills and the enjoyment of the sport
would be diminished. Cheerleading and gymnastics expose participants to the usual risk of cuts and bruises, and
other more serious risks as well. Participants often fall, sprain or break wrists, ankles, arms, legs and can suffer
more serious injuries. Traveling to and from competitions and exhibitions raises the possibilities of any manner
of transportation accidents. In any event, if you or your child is injured, medical assistance may be required
which you must pay for yourself.
I expressly agree and promise to accept and assume all of the risks, known and unknown, connected with
related activities, including but not limited to performance of stunts and use of trampolines. My participation
and that of my child is purely voluntary. No one has forced or coerced me or my child to participate. I elect for
myself and my children to participate in such activities in spite of the risks.
I hereby voluntarily release, forever discharge and agree to hold harmless and indemnify CVCTC/CVP from any
and liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way
connected with my child’s participation in CVCTC/CVP related activities.
Should CVCTC/CVP be required to incur attorney’s fees and costs to enforce this agreement, I agree to indemnify
and reimburse them for such fees and costs.
I certify that my child has health, accident and liability insurance to cover bodily injury or property damage that
may be caused or suffered while participating in this event or activity, or else I agree to bear the costs of such
injury or damage to my child. I further certify that I am willing to assume and bear the costs of all risks that may
arise or be created, directly or indirectly, through or by any such condition.
I will not quit a team at CVCTC/CVP, due to grades. School work is important, however may not interfere with
any practices. A limit of 3 absences from Sept-May are allowed, and the director has the right to remove a
cheerleader from the team if there are more than 4 absences. Parents are not allowed to use CVCTC/CVP as a
punishment for a cheerleader’s behavior.
Once a contract is signed, there are no refunds given at any time. If a parent or cheerleader breaks their
contract, then it is the responsibility of the person signed below or on the contract to continue paying the
contract in full till the end of season. Cheer season runs from June-May.

By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation or
the participation of any of my children in this activity, I may be found in a court of law to have waived my rights
to maintain a lawsuit against CVCTC/CVP on the basis of any claim from which I have released CVCTC/CVP by
signing this agreement.
9.
Once your cheerleader has made a team at CVCTC/CVP, in May, Contracts will be signed for the start of June.
I have had sufficient opportunity to read this entire document. I have read it and understand it.
I agree to be bound by its terms.

If you have any questions or concerns that need immediate attention please use the following chain of
command:
1. Head Coach
2. All Star Director
The CVCTC/CVP program maintains the right to refuse services at any time. We are allowed to remove
anyone from any team at any time.
Good sportsmanship, polite manners and a good disposition are mandatory at all practices and
competitions.
It is your responsibility to wear the appropriate practice wear to every practice.
Personal items such as cell phones, school bags, or gym bags must be taken into the gym and stored in
the lockers/cubbies in the practice area.. NO items are to be left in the gym hallways and cell phones
must be put in the blue bucket, turned off, when practice starts.
NO GOSSIP about any other teams (All Star or school). NO GOSSIP about a child on your team or
another team. NO GOSSIP about coaches and staff. It is much better to address a problem than to
listen to idle gossip.
No profanity or abusive language.
Please keep in mind the gym is not a babysitting service. We want our All Stars to use the gym as
often as possible, but no child should be dropped off at the gym unsupervised.
You must arrive at all practices, competitions or any scheduled event on time. Punctuality is a MUST.
During practice your opinion does not count. All squad and routine decisions are left to the discretion
of the coaches.
Parents are NOT allowed to stand in windows and watch a team practice. When coaches feel the time
is appropriate, the parents may watch the last 15 minutes.
Please feel free to talk to your coach about anything: remember to do it at the appropriate time. For
example, approaching a coach in the middle of a practice would not be an appropriate time.
Never post any negative comments about CVCTC/CVP on social media.
Practices may be changed or added at any time during the season. Schedules must be adjusted. Other
“activities” come 2nd to CVCTC All Star Cheer Teams.
Anyone threatening to quit or to pull their child from a team will be dismissed from the program
immediately. No refunds and the balance of the contract for the year will be charged immediately on
the credit card on file.
Only cheerleaders and coaches are allowed in the gym.
No one is allowed to yell onto the floor or try to make contact through the parent viewing area
window or doors of gym during practices or tumble classes. This is extremely distracting to all
involved.
It is the parent’s responsibility to know what is going on with your team. Check your emails and the
website regularly.
Withholding children from a practice or a competition should never be used as a form of punishment.
Athletes should be able to handle school work and All Star practices, homework is not an acceptable
excuse for missing practice.

Parents, relatives, friends and cheerleaders are never allowed to speak with competition officials for
any reason.
All CVP athletes during the season June-May, once a contract signed, are NOT allowed to visit any
other all-star facility. This would be to visit, practice or tumble.
Parents are never allowed to represent CVCTC/CVP under any circumstances concerning
accommodations, competitions or any situation.
There will be no arguing or questioning of the coaching staff’s decisions at competitions.
If a problem arises between you and a teammate or another parent, the problem will be addressed
with all parties involved at a meeting with your coach and the All Star director.
Anonymous emails/letters/texts/twitters, etc. will be ignored.
The All Star Director may change, add or subtract any rule at any time.
Cheerleaders are NOT allowed to drive themselves to or from any competitions. Even if 18 years of
age and parents are seniors in high school, a parent needs to drive the cheerleader to all events. If
there is a conflict, please ask coaches, then director.
All Cheerleaders will use the same accommodations at all out of town competitions when required to.
DUE TO CONTRACT AND “STAY TO PLAY” OBLIGATIONS ALL ATHLETES ARE
REQUIRED TO STAY AT TEAM HOTELS FOR CERTAIN COMPETITIONS, DALLAS, SUMMIT, ETC…
SEE COMPETITION SCHEDULE.
There must be a parent in every room, no exceptions.
Each cheerleader must have a chaperone at every competition. It is not your coach, Team Parent or
CVP staff’s responsibility to be your child’s chaperone.
It is your responsibility to pay the hotel directly for your room.
Bus service will be provided at NCA in Dallas. The cost of the bus is an additional fee that will be paid
by each family.
Proper traveling attire will be discussed with each team prior to an out of town competition.
All CVCTC/CVP logos and brands are protected.
All spirit wear items are to be purchased through our website or with an in house order form.
If a sponsor wants to buy an item for entire team; that order must go through CVCTC/CVP as well.
All spirit items must be approved by CVCTC/CVP.
Thank you for choosing to become a part of our program. Our hope is that this is a wonderful experience
for your entire family. However, we do know life gets in the way sometimes and we want you to know up
front some ways that your spot may be in jeopardy on your team: delinquent accounts, loss of
skills/abilities, extensive absences/tardiness, poor attitudes or injuries. If an injured athlete is replaced in
a routine, they may or may not be worked back in the routine, this will be up to the coach.
As always, if you have any questions please feel free to contact us.
LET’S GO CVP!
All Teams will have a very informative meeting during their first week of practice. This meeting will go over
the coach’s expectations of your new team. Please make plans to attend this meeting.
