2015 Dan & Company Recital Video Order Form
Video Production Services is pleased to video the 2015 Spring Recital for Dan & Company.
This years recitals will be at Rialto Center for the Arts on Sunday, May 31st at 1:00 pm
(“The Little Mermaid”) and Sunday, May 31st at 4:00 pm (“Let’s Go To The Mall”). The price
of each video will be $35.00, which includes postage to send it to you. If you want to order both
shows together, the cost is $50. You may order additional copies for relatives. The completed
videos will be mailed within 30 days of the recital. To order your video, please complete this
order form below. You can leave your completed order form at the video table or mail it to the
above address with your check made payable to Video Production Services.
This program is now available on DVD-R and on Blu-Ray discs. DVD-R’s and Blu-Ray discs
are $35.00 each. Please note that DVD-R discs will not always play on all DVD players. The
following website may be used to determine if your DVD player will work with DVD-R discs:

You can order BluRay discs, instead. There are fewer compatibility issues with Blu-Ray. If you
have a Sony PS-3 or PS-4 (Playstation) you have an excellent player for Blu-Ray discs.
You now may pay by credit card. If you mail in your order and pay with credit card, you MUST
include credit card number, expiration date, security code on card and your email address for our
credit card service to accept your payment. You can email us your order form, but you must pay
