Being a young person is not easy. Many end up down dangerous roads fraught with serious difficulties. For 22 years, the
Dave Smith Youth Treatment Centre has worked with troubled youth to help get them on a path toward a better future.
Through the generosity of people in this community, we’ve been able to provide thousands of youth and their families
with life-saving programs and care.
Kids who successfully work through their substance abuse and mental health issues are provided a second chance
at life. They are now able to dream of their future. Repairs are made to what was once broken and their families are
renewed with hope.

This fundraising hoedown promises to be barrels of fun as we transform the Horticultural Building at Lansdowne Park
into a memorable barn party on May 21, 2015.
All proceeds from the Jamboree will support Dave’s Dream – our fundraising campaign to build a new, much-needed
youth treatment centre in Ottawa.

Dave Smith’s WILD WEST JAMBOREE
SPONSORSHIP AGREEMENT
General. The sponsor hereby agrees to sponsor the event DAVE SMITH’S WILD WEST JAMBOREE FUNDRAISER FOR DAVE’S
DREAM CAMPAIGN described in the SPONSORSHIP PACKAGE, a copy of which is attached hereto as Exhibit A and is incorporated
herein by this reference. The Sponsor agrees to pay the following fees to DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION:

In consideration therefore, DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION shall provide Sponsor with the benefits
described in the SPONSOR PACKAGE. DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION may adjust such benefits,
substitute benefits or provide other benefits with the approval of the Sponsor.
Payment. Payment is due on or before _____________, 2015. Failure to pay fees when due may result in termination of this agreement.
MONTH/DAY
Limitation of Sponsorship. Unless otherwise provided, the cash and the Sponsor benefits described in this agreement are related
solely to the DAVE SMITH’S WILD WEST JAMBOREE, MAY 21, 2015. Assuming the Event is continued in 2016 the Sponsor shall
have the first right of refusal to continue Sponsor’s involvement in the Event on such terms as Sponsor and DAVE SMITH YOUTH
TREATMENT CENTRE FOUNDATION shall agree.
License and Usage. Sponsor hereby grants DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION a limited, non-exclusive
license to use Sponsor’s trade names, trademarks, service marks and other proprietary information [the “Proprietary Information”]
owned by the Sponsor. DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION agrees that the Proprietary Information will
be used only in connection with the DAVE SMITH’S WILD WEST JAMBOREE to provide the benefits set forth in the SPONSOR
PACKAGE and such other benefits as Sponsor and DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION may agree. DAVE
SMITH YOUTH TREATMENT CENTRE FOUNDATION’S use of the Proprietary Information is subject to the approval of the Sponsor’s
designated representative, which approval will not be unreasonably withheld.
Sponsor Understandings. Sponsor understands as follows: the dollar values set forth in the SPONSOR PACKAGE are estimates
based upon general sponsorship information available to DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION; the audience
multiplier is based upon estimates of DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION attendance and actual attendance
figures may vary materially. No portion of the sponsorship payment are subject to being refunded for any reason except as provided
herein. Sponsorships are not exclusive unless otherwise set forth in the SPONSOR PACKAGE.
Termination. DAVE SMITH YOUTH TREATMENT CENTRE FOUNDATION may terminate this contract on 30 days written notice. In
the event of termination for any reason other than failure to provide contracted fees or services, DAVE SMITH YOUTH TREATMENT
CENTRE FOUNDATION may substitute an event with equivalent benefits with the approval of the Sponsor or shall refund any cash
fee received from Sponsor, Sponsor shall not terminate this agreement without the approval of DAVE SMITH YOUTH TREATMENT
CENTRE FOUNDATION.
Amendments. This agreement may be amended only by a written instrument executed by both parties.
Miscellaneous. This agreement is governed by law and is binding upon all of the parties’ successors and assigns.
