The 2015 Pres. Carlos P. Garcia Provincial Scholarship Program Examination for High School Graduates will be held on the following dates, time and venues, from 9:00 AM to 11:30 AM. Date of Examination: @ April 20, 2015 Venue: - Bohol Island State University City of Tagbilaran (Applicants from ESA I) @ April 21,2015 - Talibon I Central Elementary School (Applicants from ESA II) @ April 22, 2015 - Carmen West Central Elem. School (Applicants from ESA II & ESA III) @ April 23,2015 - Jagna Central Elementary School (Applicants from ESA III) 2. This is one of the programs implemented by Gov. Edgar M. Chatto under Provincial Ordinance No. 8, series of 1996 which provides financial assistance to poor but deserving students in their pursuit for college education. 3. The following are qualified to take CPG Qualifying Examination: a. The annual income of parents is not more than P100,000.00 per annum b. Has NCAE rating which is not lower than 75% c. Applicant is not a child of municipal and provincial officials d. The following are the requirements to be submitted to the Chief Examiner before the start of the examination. a. Application letter addressed to Gov. Edgar M. Chatto through Dr. Wilfreda D. Bongalos, Schools Division Superintendent b. Xerox copy of Report Card certified by the School Principal c. Xerox copy of NCAE rating certified by the School Principal d. Certification that applicant is of good moral character and that he/she belongs to the upper 25% of the graduating class signed by the School Principal e. Tax Exemption Certificate or Certification that applicant is an indigent and that his parent's annual income is not more than One Hundred Thousand Pesos (P100,000.00) signed by the Barangay Captain/ DSWD Officer/Municipal Mayor 5. Successful examinees shall be duly notified through cell phone calls and shall prepare and submit the following additional documents to the Education Development Center, Provincial Capitol, Tagbilaran City: a. Medical Certificate signed by a Government Physician b. Certification from the Barangay Captain/Municipal Mayor that applicant is a bonafide resident of the barangay/municipality 6.
