Release Form I understand that __________________ (Entrant) will be submitting a video in the Roman Town Video Trailer Contest, sponsored by Dig-It! Games, and that the video contains a video image of the individual(s) and/or minor(s) named below and/or a sound recording of his/her voice. I hereby authorize and grant permission to the Entrant’s use of the individual(s)’ image, voice, appearance and/or name, or any portion thereof (Image), in this video. I understand that the Image may be used, published, distributed, modified, adapted, and edited for any and all lawful purposes, including for art, advertising, promotional, editorial or educational purposes of Dig-It! Games in any medium (including the Internet). In addition, I waive the right to inspect or approve the finished product wherein the Image appears, including written or electronic copy, and I waive any right to royalties or other compensation arising or related to the use of the Image. I hereby release and discharge Dig-It! Games from any and all claims, demands, and causes of action arising out of or in connection with the inclusion of the Image in the Entrant’s video. This release will be binding upon me, the minor and our heirs, legal representatives and assigns. I am eighteen years of age or older, am the parent and/or lawful guardian of the minor, and have the right and authority to grant this release on behalf of the minor.
