REGISTRATION OPEN DIVISION $400 (Compare to Men’s A/B Leagues) The open Division is considered the most competitive division of the Evergreen Pond Hockey Championships. This division is for men and women 18 years and older. Some open division teams carry former NHL legends, Division 1 hockey players, college or high school greats. Each team will be guaranteed a minimum of 3 games Saturday with the top teams advancing to the finals on Sunday. These teams may have up to 8 players with 4 on the ice at a time. The remaining two teams will compete for the championship with only one team emerging triumphant to win the bragging rights for 1 year as the Open Division Champions along with a $1,500 prize!! 35+ OPEN DIVISION $400 The 35+ Open Division is for men and women age 35 and older. Each team will be guaranteed a minimum of 3 games Saturday with the top teams advancing to the finals on Sunday. These teams may have up to 8 players with 4 on the ice at a time. The remaining two teams will compete for the championship with only one team emerging triumphant to win the bragging rights as the 35+ Open Division Champions for 1 year along with a $750 prize!! RINK RAT OPEN DIVISION $400 (Compare to Men’s C Leagues) The Rink Rat Open Division is for men and women ages 18 and older that are dedicated to the pond, but may not have the skills to compete against former NHL, college or high school greats. Each team will be guaranteed 3 games on Saturday with the top teams advancing to the finals on Sunday. These teams may have up to 8 players with 4 on the ice at one time. The remaining two teams will compete for the championship with only one team emerging triumphant to win the bragging rights as the Rink Rat Open Division Champions for 1 year along with a $750 prize!! RINK RAT AMATEUR DIVISION $350 (Compare to Men’s E & lower Leagues) The Rink Rat Division is for men and women ages 18 and older that love the game, but are strictly in the amateur category. This means players don’t participate on regular basis or at all in leagues and would rather play when there’s ice outside rather than being stuck inside on a sheet of ice. Each team will be guaranteed 3 games on Saturday with the top teams advancing to the finals on Sunday. These teams may have up to 8 players with 4 on the ice at one time. The remaining two teams will compete for the championship with only one team emerging triumphant to win the bragging rights as the Rink Rat Amateur Division Champions for 1 year along with a $500 prize!! WOMEN’S DIVISION $400 (Compare to B/C League) The Women’s Division is intended for A/B level women players and teams ready to take on some of the best female hockey players from far and near. Any C/D/E level women are encouraged to enter the Rink Rat Amateur Division. Each team will be guaranteed 3 games on Saturday with the top teams advancing to the finals on Sunday. These teams may have up to 8 players with 4 on the ice at one time. The remaining two teams will compete for the championship with only one team emerging triumphant to win the bragging rights as the Women’s Division Champions for 1 year along with a $750 prize!! Registration begins October 1 and ends on December 28. Payment by credit card and cash are preferred, checks will be accepted if received 15 days prior to the event. Team registration can be mailed in, pay over the phone with a credit card, or by PayPal at www.evergreenpondhockey.com. Team registration will not be accepted after December 28. Tournament Hours: Warming Hut will open at 5:30 am Registration and check in will be from 5:30 am to 6:45 am Tournament will begin at 7:00 am (Unless otherwise informed by staff) *TEAMS MUST BE PREPARED TO PLAY BOTH DAYS* Depending on the amount of teams, Friday games may occur as well To register by mail – Fill out form below and mail with check to: *Returning teams that register prior to December 1 will receive a $50 discount* The Evergreen Lake House 29612 Upper Bear Creek Rd. Visa/MasterCard/Amex/Discover accepted *All registrations will be confirmed via phone* Cancelation policy: If the tournament is cancelled due to weather, a refund equal to 75% of the registration fee will be returned to the team. The team may also use the full fee for the following years tournament. There must be a minimum of 20 teams registered in each division for full prize payout in that division. Teams may cancel their registration 20 days prior to the event and receive a full refund. Any cancellations 10-19 days from the start of the tournament will result in a $100 cancellation fee. There will be no refunds issued within 9 days of the tournament. Waiver and Release of Liability Clause
By signing this waiver I / we understand and acknowledge that the sport of hockey involves many
inherent risks, dangers and hazards. This Waiver and Release of Liability is signed with full knowledge of
those risks, dangers and hazards, and our agreement to accept all such risks. The Applicant and his/her
Parents/Guardians waive any and all claims that each or any of them may now or in the future have
against Evergreen Park & Recreation District, and its directors, officers, sponsors, agents, employees,
volunteers, successors and representatives, including referees, coaches (and their assistants), and
instructors for any kind whatsoever, due to any cause whatsoever, including negligence and or breach of
contract arising out of or in connection with the participation of the applicant in Evergreen Park &
Recreation District programs and or the Evergreen Pond Hockey Tournament. The Applicant and his/her
parents or Guardians warrant that the Applicant is in good health and has no medical problem or
condition, unless otherwise specified in writing. The team list shown above comprises the full and
complete list of players allowed to participate in any game. The undersigned shall be responsible for
ensuring that only these above listed players are allowed to play or be place on the Score Sheet. The
undersigned shall personally assume all legal responsibility and liability for any unauthorized player use
and shall hold harmless and indemnify Evergreen Park & Recreation District for any property damage or
personal injury occasioned by unauthorized player use.
