All healthcare professionals and frontline staff
desiring to apply for a Certified Dementia Practitioner
(CDP) from the National Council of Certified Dementia
Practitioners including, but not limited to CNAs, home
health aides, activity professionals, social workers,
nurses, rehabilitation professionals, administrators,
physicians, pharmacists, and dementia unit managers who provide direct care for dementia residents.
This seminar is designed for all healthcare providers
and any individuals who have a desire to increase
their knowledge about dementia and Alzheimer’s
disease conditions. This seminar will empower staff
and caregivers to have a better understanding of best
care practices.

Registration Information
Tuition is $150 for FADONA members and $175
for non-members. You will receive a syllabus, 3-ring
binder with course content, a CDP application, and
certificate of completion. You will receive a confirmation of your registration via email with directions to
the classroom site. Once payment is received, you
are enrolled.
Cancellations are refundable, less a $75 administration fee. Notice of cancellations must be made in
writing and received prior to one week of the seminar
date to qualify for a refund. Refunds will not be issued
to registered attendees who fail to show up and have
not given notice of cancellation. Substitutions may be
made upon approval by FADONA.
Checks with insufficient funds will be charged a
$35 service charge.
No certification will be given to a participant who
leaves early. If you are pursuing a CDP, you will
have 30 days to submit your application (additional
discounted FADONA fee of $25 — normally $100) to
NCCDP for consideration.
Completion of this course does not guarantee
certification. Please refer to NCCDP website,
www.nccdp.org, for criteria. Signature for credit card
payment authorizes FADONA to charge your credit
card for the seminar fee, including any non-refundable fees.
