Scholarship Summary
Each applicant must submit the following materials to the State 4-H Headquarters via our online
submission system by the June deadline:
 A fully completed 4-H scholarship application, available at the below link. The webpage does
not save, so please submit only once you have completed the application in its entirety:

 A personal resume consisting of 1-2 page(s), though it is recommended to keep it contained
to one (1) page. Please follow a “business professional” style resume format
 A 2-3 page Narrative Statement
 An unofficial transcript of high school/college grades, scanned
 Statement of Financial Need. ONLY required for scholarships based on financial need.
Please secure and have the following people submit a reference form to the email noted in the
instructions. You can find the form at this link: http://florida4h.org/awards/files/ReferenceForm.pdf
 One Candidate Reference Form from a school counselor, academic advisor, instructor, or
employer
 One Candidate Reference Form from your County Extension Agent
The applicant before submission to the State 4-H Headquarters should compile all of the
above materials. (The State 4-H Headquarters will not be responsible for inserting late
transcripts, recommendations, etc. into an applicant’s folder…all materials must be
submitted at one time.)
UF College of Agricultural and Life Sciences (SHARE Scholarships)
 Must be a Florida 4-H member and a graduating senior in high school during the year of
application. Students must be admitted to the University of Florida and major in an
academic program in the College of Agricultural and Life Sciences.
 Four (4) $1000 Scholarships Awarded

Fred and Mabel Barber Scholarship
 Must be a 4-H member and a graduating senior in high school during the year of
application. Can be applied to any field of study at an accredited two or four year college
or university.

T.G. Williamson Scholarship
 Must be a 4-H member and a graduating senior in high school during the year of
application. Applied to any field of study at an accredited two or four year college or
university.

Thomas Greenawalt Scholarship
 Must be a 4-H member and a graduating senior in high school during the year of
application. Applied to any field of study at an accredited two or four year college or
university.
 One (1) $400 Scholarship Awarded

Platt Memorial Scholarship
 Must be a former 4-H member and a sophomore or junior in college during the year of
application. Must be enrolled at the University of Florida or Florida State University.

Ruth Milton 4-H Family, Youth, and Consumer Sciences Scholarship
 Must be a Florida 4-H member or former member who has participated in 4-H Family,
Youth, or Consumer Sciences projects for at least three (3) years and has a financial
need.
 A resident of Florida who is registered in or has been admitted to a college or university
offering major work in Family, Youth, or Consumer Sciences; or an incoming freshman
pursuing a focus in Family, Youth, or Consumer Sciences.

Alden Hilliker Memorial Scholarship
 Must be a 4-H member and graduating senior in high school during the year of
application. Applied to any field of study at an accredited two or four-year college or
university.

Nettie Ruth Brown Scholarship
 Must be a 4-H member who is a graduating senior in high school who has been
accepted to the University of Florida. Primary selection will be given to those members,
first from St. Johns County, then from Putnam County, and then from District VI. If no
suitable candidate is found, the selection process will be opened to 4-H members
statewide.
 Preference will be given to members who have distinguished themselves scholastically,
shown leadership potential as exhibited by their academic record and members who
have exhibited achievement and/or leadership abilities within 4-H; the greatest emphasis
will be placed on academic excellence and preference for those with financial need.

Norman Cox Scholarship
 Must be or have been a Lee County 4-H member: a high school senior, freshman, or
sophomore in college during the year of application who is accepted to an accredited
two or four year college or university in Florida or Abraham Baldwin Agricultural College
in Georgia
 The applicant must have been a member of the Florida 4-H program for at least one
year and have a need for financial assistance.

Terry Floyd Statewide Scholarship
 Must be a 4-H member and a graduating senior in high school during the year of
application. Applied to any field of study at an accredited two or four-year college or
university.

Dr. Freddie Johnson 4-H Scholarship
 Must be a 4-H member and a graduating senior in high school in Pasco, Hernando,
Citrus, Polk, Sumter, Marion, Orange, Osceola, Lake, Brevard, Seminole, or Volusia
County. Applied to any field of study at an accredited two or four-year college or
university. This award is based on merit.

Jared Padgett 4-H Excellence Scholarship
 Must be an active 4-H member and a graduating senior in high school during the year of
application. Applied to any field of study at an accredited two or four year college or
university.
 One (1) $250 scholarship awarded

Florida 4-H Dog Project Annual Scholarship
 Must be or have been a 4-H member; all applicants must have completed a minimum of
one year in the Florida 4-H dog project; a graduating high school senior,
freshman, sophomore, or junior in college during the year of the application with a
background in working with animals. Preference will be given to youth pursuing animal
related studies including pre-veterinary sciences. Special consideration will be given
to applicants with extensive participation in the 4-H Dog Project.
 One (1) $1000 Scholarship Awarded. If no eligible applicants selected, the scholarship
will be carried over to the following 4-H program year.
 This scholarship is presented in memory of Gordon Ruble, webmaster and member of
the Florida 4-H Dog Project Committee and sponsored by the Florida 4-H Dog Project
Volunteer Committee
Tampa Bay Kennel Dog Scholarship
 Must be a current or former 4-H member with a background in working with animals,
beginning his or her freshman, sophomore, junior, or senior year. Preference will be
given to youth pursuing animal related studies including pre-veterinary sciences.
 Preference will be given to former 4-H’ers from Central Florida counties (Hillsborough,
Pinellas, Pasco, Hernando, Polk, Sarasota, and Manatee). If none have applied, the
scholarship will be open to 4-H’ers of any county. Financial need will be considered
based on parents and /or applicant’s income and situation.
 One (1) $500 scholarship awarded

Chuck and Linda Syfrett Scholarship
 Must be a 4-H member and graduating senior in high school during the year of
application that will be attending the University of Florida.
 One (1) $1,000 scholarship awarded

Equine Scholarships, below, require the following materials
in addition to the Florida 4-H Senior Portfolio:

Current 4-H horse project record book.
Outline summary of horse activities outside 4-H (not to exceed one page, i.e.,
scouts, breed associations, horse organizations).
Horseman of the Year
 Must have completed at least junior year of high school and have completed at least two
years in the Horse project including current year. All applicants are limited to two years
of eligibility/participation. Previous first place winners will be ineligible to compete. First
and second runner’s up and all other applicants will be able to compete. The interview
will be conducted at the State 4-H Horse Events.

Dallas C. Osborne 4-H Horsemanship Scholarship
 4-H members who are participants of the Area B Horse program including Alachua,
Baker, Bradford, Clay, Columbia, Dixie, Duval, Flagler, Gilchrist, Hamilton, Jefferson,
Lafayette, Levy, Madison, Nassau, Putnam, St. Johns, Suwannee, Taylor, Union
Counties.
 Award will be judged according to the criteria used for the 4-H Horseman of the Year
award. Interested applicants should complete all requirements for that award. Selection
will be done via the 4-H Horseman of the Year process and committee.

Procedure for Requesting Scholarship Payment
Payment of scholarship funds is made directly to the accredited college or university or technical/trade
school on behalf of the recipient. In the event of special education needs, payment of scholarship awards
can be directed to the recipient upon prior approval of the Associate Dean for 4-H. Under these conditions
an IRS Form 1099-MISC will be filed with the Internal Revenue Service and with you for the calendar year
in which it is received.
Winner must submit the following to the State 4-H Headquarters:
• College/school name, contact person's name and address
• Enrollment verification
• Copy of thank you letter written to donor
• Social Security Number
• Student ID (if used for registration way at designated higher educational institution)
Conditions Governing State Scholarships
A scholarship shall be used within one (1) year of the date of the award. In the event that the scholarship
is not used within the year of limitation, the winner must present in writing to the Assistant Dean for 4-H a
request for time extension. Failure to do so will result in a forfeiture of the scholarship.
