2015 Poetry Contest Rules and Entry Forms Rules 1. 2. 3. 4. Only children in grades 1 through 12 may submit work. Every poem MUST be accompanied by an entry form WITH CONTACT INFORMATION. Multiple entries are allowed as long as every entry is accompanied by an entry form. Poems must be turned in to the 1st Floor Teen Ask Us! Desk or 2nd Floor Ask Us! Desk by March 16, 2015. Frequently Asked Questions How are winners chosen? Our librarians look at every entry and whittle the poems down to 20 or 30 finalists for each age division. They send the finalists’ entries to this year’s judge, Karla Morton. She chooses three equal winners in each age category. There is no first, second, or third place. What are the prizes? Winners will be invited to attend a special reception in their honor where they will receive a certificate and recognition from the community. Winners will also receive a keepsake book containing all of the winning poems. Can I get my poem back? Once the poem is submitted to us, we cannot return it. Please make copies of your poem and do not send the original. How/When will I be notified if I’ve won? The library will contact you by phone, through your teacher, or through email. All winners will receive a special invitation to the reception through email. Winners will be notified by April 8, 2015. If you are not selected as a winner, you will NOT be notified. How many poems can I submit? As many as you want! But every poem submitted must have an attached entry form WITH CONTACT INFORMATION. No bulk submissions with one form. Any poem submitted without an entry form attached will be automatically disqualified. __ __ __ __ __ __ __ __ __ __ __ __ __ __ __ __ __ __ __ __ __ Poet’s Name _____________________________________________ Age/Grade ______________________________________________ Address ________________________________________________ Phone/Email ____________________________________________
