signatures to the FST Registrar. Do not attempt to register through USD “One-Stop” or USD registrar’s office.
1. Select the course you’re interested in from the list of approved courses (found on the FST
portal and website) and consult with your advisor.
2. Contact the USD course instructor to see if there is space available and to get permission to
enroll. Email approval in lieu of signature is fine (attach email to this form)
3. Get signature of the FST Academic Dean.
4. Bring the complete, signed form to the FST registrar who will register you for the course.
