Scholarship Information !
1. Five Commendable students will be awarded a Kupa’a Scholarship for the 2014-2015
school year that’s worth $1000.00 each. Recipients are selected from Maui, Kauai and
Oahu.
!
2. You can find application forms on Hawaii Speed and Quickness’ website
(hawaiispeedandquickness.com).
!
3. Completed application forms, essays and letters of recommendation must be received by
Hawaii Speed and Quickness by 4 p.m. on May 27th, 2015. !
To Apply,You Must:
!
1. Be a high school senior graduating from a State of Hawaii high school.
!
2. Have a minimum cumulative grade point average (GPA) of 2.0.
!
3. Have played on at least one school athletic team in a league sanctioned sport during at
least one of your high school years.
!
4. Have participated in community service projects in addition to school activities.
!
Notification of Winners
!
Once you’ve been chosen as an Hawaii Speed and Quickness Kupa’a Scholarship recipient,
we’ll send you an award letter.

1. Send your completed application, essay, and letters of recommendation to the following
address by 4 p.m. on May 27, 2015.

2. Include your current home address and mailing address so we can contact you if you win.

3. List all the teams you’ve participated on. If you’ve been to two or more schools, include
each school and which sports you participated in.

4. We want to hear how you’ve helped your community. Please include details.
!
5. If you have academic awards, make sure you include those too.
!
6. Your GPA must be verified by a report card or an authorized member of the school faculty.
!
7. Include letters of recommendation from at least one high school teacher or counselor
commenting on your determination, work ethic, dedication, compassion, academics,
sportsmanship and personal character.

8. Please include a current photo.
