HNHB CCAC Nurses – Putting Patients First
When we find ourselves in need of health care, a nurse is usually our first contact. As Canada’s
largest group of health professionals, nurses can be found in patients’ homes, in the community
at schools or at work, in palliative care teams and hospices, nursing care centres, hospital
wards and emergency rooms, and on the phone or online giving advice.
The Hamilton Niagara Haldimand Brant Community Care Access Centre (HNHB CCAC) is
pleased to honour our hard-working, dedicated nurses for their significant contribution to
improving the quality of health care throughout Ontario and across Canada.
Through their dedication to safe, high quality patient care, CCAC nurses and nurses employed
by our home care provider partners ensures that people of all ages, at every stage of life get the
right care, in the right place, at the right time.
CCAC is extremely fortunate to have many trained nurses serving as Care Coordinators. They
work with patients and families to develop individualized care plans for people designed to
deliver the care and health supports they need to remain safely at home. We also have
specially-trained nurses who work with patients to support their recovery and well-being.
Rapid Response Nurses and Nurse Practitioners support adults and children with high care
needs as they return home after a hospital stay. Services may include a home visit within 24
hours of hospital discharge, medication review, comprehensive nursing assessment, patient and
family education and ensuring follow-up appointments are scheduled.
Hospice and Palliative Care Nurse Practitioners and Clinical Nurse Specialists act as a
bridge to connect, support and care for patients who have a life-limiting illness. Our nurses help
people and families understand their options and provide support so that, as a patient reaches
closer to the end of their life, they are able to live their remaining days comfortably at home for
as long as possible.
Mental Health and Addiction Nurses support kids and teens at school focusing on the
success of those with mental health or addiction issues, helping them thrive in school or
successfully transition back to school after a hospital stay.
