Please ensure that you answer each section of the Entry Form.
Please provide at least ten (10) high resolution images (minimum 1MB).

Applications must be received by 5pm Tuesday 7th April 2015.
The project must have been conducted in South East Queensland which includes the area from
the Queensland/New South Wales border, north to Noosa and west to the Great Dividing
Range.
The project must have produced major results between 1 January and 31 December 2014,
except for the Waterways Champion Award, which must show a sustained contribution over the
past three years.
Past entrants and winners are eligible to reapply in 2015, provided their project demonstrates
substantial progress and achievements between 1 January and 31 December 2014. Past
winners of the Waterways Champion Award are not able to reapply for this award.
Information and images contained in the application or supplied as video may be reproduced
in Healthy Waterways’ promotion and publicity materials.
Entries and all supporting documents will not be returned to entrants.
Entries are judged by an independent panel of industry experts and their decision is final.
The Minister’s Grand Prize can only be awarded to a group or organisation. Individuals, as well
as entrants in the government award category, are not eligible.
All finalists determined by the judging panel will be eligible for the People’s Choice Award.
A project may be entered in multiple categories but it will only be recognised as a finalist in one
category.
Healthy Waterways and the judging panel reserve the right to determine which category the
entrant will be recognised under.

Category of Entry
Of the 12 Healthy Waterways award categories, five are open to individuals who are working
towards achieving healthy waterways. This Entry Form is for all Individual award categories, including
the Waterways Champion Award. Please note that ‘Individual’ entries, as well as entrants in the
government award category are not eligible for the Minister’s Grand Prize.

Next Steps
Thank you for taking the time to submit an entry into the 2015 Healthy Waterways Awards. Your
entry will be judged by a panel of experts based on the material submitted. Finalists will be
announced on Tuesday12 May 2015 in conjunction with the launch of the People’s Choice Award
Campaign which calls on the public to vote for their favourite finalist.
Join us for the announcement of the award winners at the 2015 Healthy Waterways Awards Gala
Dinner. This is an exclusive opportunity to honour our waterway champions, gain insight into the
many innovative waterway health projects being undertaken in the region, as well as meet with
community leaders and industry officials. The ticket price includes a two course dinner, drinks and
entertainment.
