Tuesday
Depart Your Home City - Arrive Honolulu, Hawaii (early evening)
Upon arrival at the Honolulu International Airport you will receive a traditional Hawaiian Lei Greeting and be taken directly to
the hotel. An Honor Choir USA representative will greet you at the hotel and assist you with express check-in. You will then
have the remainder of the afternoon and evening to explore the Waikiki Beach area. A security meeting will close the first day.
Wednesday Sightseeing Tour and Welcome Party
Today will be a full day of sightseeing and fun. Don’t forget your swimsuit. The tour bus will leave the hotel at 7:30 am for
Pearl Harbor. You will visit the U.S.S. Arizona Memorial, a very solemn experience and one that most will never forget. Next
is the Aloha Flea Market at Aloha Stadium where you will find great shopping. From there the tour bus will travel to the Pali
Lookout and more sightseeing along the coast. We will stop and enjoy swimming, sun, and surf at one of the beaches. We will
return for an evening on Waikiki Beach for your welcome pizza party and hula dancing lessons. Then, your first rehearsal!
Thursday Day of rehearsal and leisure
From 9 until 11am and from 9 until 11pm you will have choir rehearsals. You will have the day to explore the island of Oahu.
You may want to take an optional sightseeing excursion such as snorkeling at Hanauma Bay, parasailing, swimming, surfing or
maybe climb to the top of the Diamond Head Crater. Whatever your choice...this day will be one to remember.
Friday
Day of rehearsal and leisure
Once again, the morning begins with choir rehearsal from 9:00 until 11:00am and again at 9pm. You will have the day free to
experience all Honolulu has to offer. See your Honor Choir tour guide for a list of activities to enjoy.
Saturday
Pearl Harbor, Luau and Performances
Today we tour Pearl Harbor and perform on the USS Missouri and travel to the North Shore for a performance at The
Polynesian Cultural Center. We will tour the center, perform, and enjoy an authentic Hawaiian Luau.
Sunday
Performances
This morning we will perform at Hawaii’s oldest church, Kawaiaha’o Church. The service begins at 9:00 A.M. We will then
go across the street and see the Iolani Palace once the home of Hawaiian royalty. We go from the palace to the mall for lunch
and performance. This is one of the largest malls in the USA.
Monday
More Fun and then Travel Home
You and your group will have this morning and afternoon free to get in one last swim or maybe take the surfing lesson you've
been putting off all week. We depart for the airport and our flight home this afternoon.
Tuesday
Arrive in Your Home City
You arrive home this afternoon after an enjoyable experience in Hawaii with your new friends and a lifetime of memories!

Congratulations on being selected to participate in the Honor Choir USA Aloha Tour . You will need to follow
the directions below to enroll in our program. Please fill-out and sign (if applicable) each of the forms listed
below. Please do not leave any field blank on a form, as this will delay your acceptance into our program. If
you have any questions about any of the enrollment forms or about the Aloha 2015 Students Tour, please don’t
hesitate to call our office.
After you have filled out each form, and have signed and notarized where appropriate, please mail all forms
with a deposit check of $300 for each traveler to our office at:

balance and payment schedule. Payments are due in our office on the 15th of each
month. If a payment is received after the 25th of the month due, your account will be
assessed a $25 Late Fee. Your balance must be paid in full by April 15th, 2015.

Terms and Conditions
Honor Choir USA, LLC is hereafter referred to as the “Company.” The Company acts only as agent for its suppliers and contractors providing
transportation, accommodation and other related travel services. The company assumes no responsibility or liability for any injury, death, damage,
loss, or accident with any services resulting directly or indirectly from acts of God, strikes, government regulations, thefts, failure of any means,
discrepancies or changes over which it has no control.
The Company applies all reasonable checks and measures in utmost good faith to ensure that details in the itinerary are carried out exactly as
specified. However, changes, including but not limited to, substitution of hotels of equal or similar quality and air carriers, may be made out
of necessity. The Company also reserves the right to, at its sole discretion, change the dates and or location of an event and or tour.
In case of dispute or litigation between a tour member and an air carrier, the passenger contract issued together with the air ticket by the air
carrier shall constitute the sole contract between the airlines and the passenger; the Company shall not be responsible for any delays,
substitutions of equipment or any act of omission whatsoever by the carrier, its agent, and employees. In case of any type of dispute or
litigation with the Company (Honor Choir) or its employees or its agents, it is understood that any type of dispute or litigation shall be
governed by the laws of the State of Oklahoma and the undersigned agrees and hereby voluntarily submits to the jurisdiction and venue of
Ottawa County, Oklahoma.
Regardless of the reason, unused services during the tour, including hotel accommodation, ground transportation, domestic or international
flights, meals, and sightseeing are neither refundable nor exchangeable.
Baggage and personal effects are the passenger’s own risks and it is the responsibility of the passenger to obtain insurance against loss or
damage. Baggage is limited to two pieces, per passenger, weighing a maximum of 50lbs each. All baggage charges are the responsibility of
the passenger. Many airlines are only allowing one piece of luggage. Call your airline for specific details.
The Company and the operators involved in this program shall be exempt from all liability for losses or additional expenses in respect or any
detention, delay, loss, damage, sickness, or injury, irrespective of the cause, including strikes, war, quarantine and whether or not directly or
indirectly caused by any service provided or by a service provider, or by any property used or utilized by a service provider. Furthermore
the Company shall be exempt from all liability for losses related to any sightseeing activity including but not limited to snorkeling,
parasailing, surfing, skydiving, horseback riding, swimming, use of mopeds, bicycles, and mini cars.
Any disability requiring special attention must be reported to the Company at the time the reservation is made. The Company will make
reasonable attempts to accommodate the special needs of disabled tour participants, but it is not responsible in the event it is unable to do so,
nor is it responsible for any denial of services by carriers, hotels, restaurants, or other independent suppliers. We regret that we cannot
provide individual assistance to a tour member for walking, dining, getting on and off motor coaches and other transportation vehicles, or
other personal needs. Travelers who need such assistance must be accompanied by a qualified companion.
For the comfort and safety of our participants smoking is not allowed on any sightseeing tours or at any activity organized by the Company.
Airline tickets and all travel documents, will be sent regular ground delivery two to four weeks prior to departure, providing full payment has
been received.
Not included in the tour price: laundry services, food, beverages, optional sightseeing excursions, all items of a personal nature, and tips to
the hotel staff, tour director, and drivers.
The Company reserves the right to accept or reject any person as a tour participant, to expel any tour participant from the tour, to make
changes in the itinerary whenever the Company deems it necessary for the comfort, convenience, or safety of the tour participants, and to
cancel a tour at any time. If your account becomes more than 30 days past due, your trip will be subject to cancellation and no refund will be
issued. All claims against the Company must be filed in writing within 10 days after completion of the tour.
Reservation with the Company confirms that the client has read, understood and agreed to each and all of the above terms and conditions.

We, being the “Student” and the “Parent or Guardian” of said student, fully understand and willingly agree to
abide by the following conditions set forth by Honor Choir USA for participation in this event:

_____ I will be courteous, polite, respectful and friendly to my director, other choir members, on site Honor Choir
representatives and any other persons that I may come in contact with while on tour. I will represent my family,
school, and community in a positive manner at all times.
_____ I will comply with all rules and regulations set forth by the on site staff of Honor Choir and my accompanying
director.
_____ I understand I am never to go anywhere without at least three (3) accompanying members of my traveling party,
and without the prior knowledge of my accompanying coach.
_____ I will obey all laws, statutes, rules and regulation of the country, state, hotel, restaurant, transportation provider,
school or other public places I may visit.
_____ I will not throw any object from my balcony or window.
_____ I will not use any type of alcoholic beverage, illegal drug or substance at any time during the Honor Choir
tour.
_____ I will not be in a hotel room with a member of the opposite sex, or engage in any sexual activity at any time during
the trip.
_____ I understand that any violation of this Code of Conduct will result in any or all of the following solutions based
solely on the judgment of the onsite Honor Choir staff representative(s) and the Directors:
1) Notification of my Parents
2) Immediate return home on the next available flight at my own expense

THIS WAIVER EFFECTS YOUR FUTURE RIGHTS. PLEASE READ CAREFULLY.
I, the undersigned, do hereby acknowledge that I am the parent or legal guardian of the Minor/Ward set out below and
have fully read and understand this document and agree to indemnify and hold harmless, Honor Choir L.L.C., its agents,
employees, and assigns from any and all injury or loss of life whether to myself, my child, my ward, my heirs (including my
spouse, if any), or my property that may occur in connection with the Honor Choir USA L.L.C. Summer Choir Tour. I further
agree and waive any and all rights and claims that I, my child, my ward or my heirs (including my spouse, if any) may have
against Honor Choir USA L.L.C. with regard to all liabilities (except those resulting from Honor Choir USA, L.L.C. gross
negligence or willful misconduct) losses, damages, costs and expenses (including attorney’s fees) arising or resulting directly or
indirectly from, Honor Choir USA, L.L.C. I further acknowledge that said waiver has been signed at my own volition without
any coercion, promise, or threat of any kind to myself, my friends or my family.
I further agree and consent, that in the unlikely event of an accident, injury or illness concerning the said Minor/Ward,
Honor Choir USA L.L.C., acting by and through its adult representatives, will use its best effort to contact the parent(s) or legal
guardian(s) of Minor/Ward as soon as is reasonably possible. In the event the parent(s) or legal guardian(s) is not available, or if
emergency care or treatment is required, Honor Choir USA L.L.C., acting by and through its adult representatives who have the
actual care, control, and possession of Minor/Ward, shall be and hereby are authorized by this written document to secure and
consent to such x-ray examination, anesthetic, medical or surgical diagnosis or treatment, and hospital care to be rendered to
Minor/Ward under the general or special supervision and upon the advice of an athletic trainer, licensed physician, surgeon, or
dentist as they in their sole discretion shall deem necessary for the treatment of the accident or illness. I further agree, that I shall
assume full responsibility for all medical bills, doctor bills, hospital bills, paramedic bills and other bills related to the accident or
illness of Minor/Ward and hereby authorize Honor Choir USA, L.L.C. to provide the necessary insurance information to be
used.
I further agree and consent to the Minor/Ward to rehearse and perform in the Honor Choir USA L.L.C. Summer Choir
Tour. I also understand that as a participant in the Honor Choir USA L.L.C. Summer Choir Tour, the Minor/Ward may be
photographed or videotaped during the activities and events and that these photos or videos may be used in promotional
materials by Honor Choir USA L.L.C. without compensation.
