School Nurse
Your school has partnered with the Barren River District
Health Department so a Registered Nurse can be there
to support the health
needs of your child and your family.
Nursing services are free to you.

Your school nurse can help you meet these
requirements for 6th grade entry:
• Well Child Exam (for kids with Medicaid, or
who are uninsured)
• Immunizations
To schedule any of these services, call the
school and ask for the school nurse. If your child needs
immunizations (shots), you will need to bring his/her
immunization record with you if it isn’t on file at school.
Other School Nurse Services
School nurses are very helpful for children
and families who have health problems and
concerns. Our goal is to help your child
stay well and ready for school success.
We can help you access needed health
services as well.
Dental Health Services
The Barren River District Health
Department also offers a traveling
Dental Services team. They can provide
exams, cleanings, and some treatment services.
When completing school enrollment papers, be sure to
complete both the School Nurse and Dental Services
Permission Forms if you want to take advantage of them.

Q. What health services does
Kentucky law require for
children entering 6th grade?
Health Exam (school physical)
You must bring proof of a preventive
health exam conducted within
one year prior to entering into the
6th grade. The Exam Form is
available from your School Nurse ,
School Counselor, or at
http://education.ky.gov/districts/SHS/Pages/Health-Forms.aspx
Q. Which immunizations does Kentucky
law are require for 6th grade entry?
• Tdap or TD (tetanus and diptheria) –
a booster dose
• MCV or MPSV (Meningitis) – first dose
• Varicella (chicken pox) – second dose
Note: Not required if your child has proof of
having had two doses OR You have proof that
your child has had chicken pox, documented
by a healthcare provider.
Q. What other health services do you
recommend to help my child be
ready for success in 6th grade?
Q. What types of family support are
most helpful for my 6th grader’s
school success?
Eye (Vision) Checkup
Your child should have regular vision or
eye exams. Being able to see well is important
for school success. Now is a good time to take
your child to an optometrist or ophthalmologist.
A Regular Bedtime
No child can do well in school without
getting enough sleep. Two weeks
before school starts in the fall, begin
keeping a routine bedtime for your
family – and sticking to it!
Dental Exam and Cleaning
We recommend this every 6 months for all
children. Students cannot learn well
if they are in pain from dental problems.
And protecting teeth now will help your
child throughout his or her lifetime!
HPV Vaccine
The human papillomavirus can cause cancer in men and
women. Now is the recommended time for this vaccine for
boys and girls. Call your doctor or the health department.
Good Nutrition
Food and drinks are the fuel your child’s
body – and mind – run on. Serve plenty of
fruits and vegetables, avoid sugary
drinks, and stay away from caffeine.
Homework Help and Routines
Help your child to succeed by setting
regular times for homework. Turn off
the TV, computer and cell phones!
