Discuss Dual Enrollment at Florida SouthWestern State College with parents. 2. Meet with Dual Enrollment Coordinator* to determine eligibility. IF AFTER March 3, SKIP STEPS 4-7 AND SEE *** AT THE BOTTOM OF THIS PAGE. 4. Complete Student Information sheet and Dual Enrollment Agreement. (Include both student and parent signatures.) 5. Complete an online application before March 6 if you would like to take the PERT at Baker. You can access the application at http://www.fsw.edu/admissions/dual. Click on the “admissions application” link. You will create a new account and select the Dual Enrollment application. The application takes about two weeks to process, so be aware of your upcoming deadlines. 6. Bring in a copy of your FSW ID to Mrs. d’Albenas. If you have already met minimum GPA and test scores, skip to step 8. If you need a minimum test score, go to step 7. 7. Take the PERT at Baker High School. Time and location will be provided to all students who bring in an FSW ID, have met the minimum GPA requirement, and return a signed PERT Referral Form. If passing scores are met, see step 8 on page two of this checklist. Dual Enrollment Coordinator is Mrs. d’Albenas. Some Dual Enrollment math courses require the higher test score. Students who are unable to attend the Dual Enrollment meeting must see Mrs. d’Albenas to: Pick up a Student Information sheet and Dual Enrollment Agreement. Submit a completed application online. Turn in a complete set of ACT and/or SAT scores to the FSW Testing Center or make an appointment to take the PERT. Be sure to see Mrs. d’Albenas beforehand to pick up a PERT Referral Form. Go onto FSW’s website to identify specific courses you want to take. See Mrs. d’Albenas for step-by-step instructions. 9. On April 24, during 4th block and after school, report to office 030 to complete the Accelerated High School Registration Approval form. 10. By May 22, take completed Accelerated High School Registration Approval form to FSW Registration in person to register for classes (classes fill quickly so earlier is better). The first semester you turn in an Accelerated High School Registration Approval form, you will also need to get your FSW ID card; this will be your pass to and from Baker campus, as well as your library card at FSW. 11. By May 22, bring a copy of your FSW schedule to Mrs. d’Albenas so that your Baker schedule may be adjusted and textbooks ordered. Until the FSW schedule is received, students will maintain a full schedule at Baker and be responsible for all work and attendance in those Baker classes. 1 FSW course at FSW or online = 1 Baker class 4 FSW courses at FSW or online = Early Admissions = no classes at Baker Research class may be available to work on online classes during the school day It is crucial that you sign up for classes by May 22 and give Mrs. d’Albenas a copy of your FSW schedule by May 22 to finalize your Baker schedule and order textbooks. The Guidance Office is closed over the break. Any FSW schedules received after May 22 WILL cause a delay in receiving your textbooks for Fall classes. 12. IMPORTANT! If you decide to drop a class, you MUST notify the Dual Enrollment Counselor immediately to complete required paperwork. Failure to complete this paperwork will likely result in a failing grade for both the college course and on your high school transcript. You may not make any changes to your FSW schedule after August 31 (the last day of add/drop). 13. In August (dates to be determined), report to Baker office #014 to check out FSW textbooks. A schedule needs to be received by Mrs. d’Albenas prior to May 22. Baker will provide the information regarding course information and books needed to the district. Those books will be provided by the Lee County School District, delivered to Baker, and checked out to students. Any delay in receiving your schedule or any changes to your schedule during the add/drop period WILL result in a delay in ordering your textbooks. 14. Attend all classes, complete all assignments, and STUDY to the best of your ability. Remember, Dual Enrollment classes affect two GPAs---high school & college. You may no longer withdraw from classes at FSW unless approved by the School District of Lee County. It is VERY IMPORTANT that you check your email on a regular basis and stay in contact with your counselor so that you are up to date on important Baker and DE information. 15. Students planning to take Dual Enrollment Classes in Spring 2016 should listen for announcements and check the Baker Dual Enrollment website for registration dates. (REMINDER: to remain eligible to continue with Dual Enrollment, students must maintain a “C” average or better in college courses.) 16. Return ALL FSW textbooks to the CRC as soon as you have completed your final exams in December. Failure to do so will result in you being placed on the Fees and Fines list at Baker.
