We are eager to extend you a personal invitation to partner with us as we seek to blot out
MELANOMA CANCER! Rated as the fastest growing cancer in the U.S. and world, melanoma has
increased 200% since the 1970's. Current statistics indicate 1 in 50 will develop this highly invasive
and often deadly cancer, but these numbers continue to rise!
The iCureMelanoma 5k is dedicated to waging war on melanoma cancer through public
awareness campaigns, free skin cancer screenings, and raising much needed research revenue
to fund local clinical trials and aggressive melanoma research! Once again, Full Spectrum
Dermatology has signed on as the residing Presenting Sponsor and will supply the medical teams
necessary in order to offer FREE SKIN CANCER SCREENINGS to each 5k registrant!
Our 5k and Community Vendor Fair will take place on Saturday, May 2nd, 2015 at Laguna Road
Elementary School in Fullerton, CA. For your convenience, we've attached a listing of both

I understand that my consent to these provisions is given in consideration of the acceptance of this registration and for being permitted to participate in this event. I
am a voluntary participant in this event and in good physical condition. I know that this event is a potentially hazardous activity and I hereby assume full and complete
responsibility for any injury or accident which may occur on the premises of this event, and hereby release and hold harmless and covenant not to file suit against
MelanomaCure.org, its local affiliates, Dr. William P. Baugh or Full Spectrum Dermatology and any affiliated individuals from any loss, liability, or claims I may have
arising out of my participation in this event, including personal injury, or damage suffered by me or others, whether same may be caused by falls, contact with
participants, conditions of this course, negligence of this release or otherwise. If I do not follow the rules of this event, I understand that this Waiver and Release may
be stored electronically and agree that a copy is authentic and admissible as evidence in any future dispute or proceeding.

Exhibitor agrees to hold MelanomaCure.org, Full Spectrum Dermatology, Laguna Road Elementary School and the City of Fullerton blameless, and to reimburse for any
judgments, settlements, costs, or other expenses (including legal fees) to which they may be subject for any injury, damage, loss, or harm resulting from or occurring
to anyone (including visitors) which may result directly from or in connection with such exhibit or the use of the rented space by the exhibitor, its employees or
representatives.
