Typical starting parameters: Power level 40; 40oC and 4sec, and increase gradually on the patient’s initial session the temperature, occasionally RF power level, as tolerated. RF Power Level o For areas of thin fat, lower the RF power level. e.g. 30 on arm, 40 on thin abdomen, vs. 50 on thick abdomen. o RF power is synchronized with the vacuum and is disabled when there is no vacuum, with a warning message “Vacuum” appearing on the interface. Cut-off Temperature o The higher the temperature within limits of tolerance, the better results. o When temperature of < 41oC is used, treatment time should be increased from a minimum of 10 min/zone. Pulse Width o A gradual heating is recommended (3-4sec). A short pulse width (2sec) will increase the number of passes and prolong the time to reach the cut-off temperature. However, it may be used on thin fat areas, like arms that bruise easily. Reducing the pulse width of RF also reduces the vacuum duration for less bruising. A long pulse width (7sec) will heat the skin too fast and will decrease the tolerance to RF. Pre-treatment Clean the handpiece with alcohol 70% or other disinfectant. Use alcohol-soaked cotton buds to clean inside the handpiece cavity. Patients should stop anticoagulants for 7-14 days prior treatment to avoid bruising, if medically permitted. Ask the patient to come with clean and dry skin. Mark the treatment zones while patient is standing. Treatment zone should be 4-8 hand piece vacuum-chamber prints (~1/4 of abdomen). Average size zone is 10X15cm or the size of a large hand palm. In smaller zones, decrease the RF power level. Treatment Procedure The use of gel, water- or fat-based preparations like creams and lotions is contraindicated! Assume full contact of hand piece with skin with only a slight pressure to enable maximal vacuum suction and deeper treatment. DO604729B Use your free hand to flatten and stretch treatment area. Press the foot switch and initiate the RF current and suction, and heat the skin for 2-7sec. Move to adjacent site with 10-20% overlap from previous site and heat for 2-7sec. Continue to treat the treatment zone in this fashion for ~2-4 passes until you raise the skin temperature in the zone to cut-off temperature. For the next 10min move around the zone evenly, maintaining cut-off temperature. Please note that time is measured from the minute the cut-off temperature is reached. Excessive heat sensation after a few passes may call for a faster movement, reduced pulse width, reduced RF power level and last, reduced cut-off temperature, in this sequence. After 10min move to the next zone, usually left or right side, and perform the same steps. Some patients who cannot tolerate the heat for full 10min, may have the treatment divided to 5+5min, allowing the skin to cool down. One zone is then treated for 5min and the other zone (left or right) is treated for 5min. After treating the second zone for 5min return to the first zone and complete additional 5min and repeat treatment of the second zone. Each zone is treated for a total time of 10min. In odd areas like navel area, pause for 5min between repeated treatments. In odd areas like navel area, allow skin cooling between repeated treatments, especially in small areas. An abdomen with 4 treatment zones may take ~40min, according to area size and parameters. Typical response to treatment is erythema, slight edema and a tighter skin look. Post-treatment Avoid very hot water for 2 days post treatment. Avoid scrubbing and scratching the treated area. Moisturize the skin. Clean the handpiece as described in Pretreatment. Treatment Schedule The number of treatment sessions is typically 6, performed once a week. Some patients may get satisfactory results after 4 sessions. Some patients may need 2-4 additional sessions and very few do not respond to treatment. Maintenance treatment consists of 1 session every 3-6 months or more according to individual response. Results Tightening of the skin is very consistent after treatment. Circumference reduction after a series of treatments is quite consistent with an average 3-4cm reduction on the abdomen and 2-3cm on the thighs. Cellulite reduction of 40-60% is observed in most patients. Tips Erythema or purpura are a common response and resolve by themselves after 1-3 days. Keep treated arm bent and supported during treatment to avoid stimulating the superficial arm nerve. DO604729B
