Lady Buffs Basketball Camp

Dates: Monday, June 1st – Thursday, June 4th 2015
Ages: Grades K-8 (grade entering in fall 2015)
Time: 9:00 a.m. - 3:00pm
Lunch: 12:00 (Pack Lunch or Eat a free school lunch - Drinks / Snacks Available for Purchase)

1 Child - $70 / 2 Children - $120 / 3 Children - $170

All participants who attend 3 days or more will receive a camp T-shirt. Prizes and awards will
be given throughout the week. First 50 campers to pre-register with a $10 deposit will
receive a regulation size basketball. You may also register at the door.

Supervision: During the instruction and practice sessions, the campers are under the close
supervision of the coaching and camp staff.

School Basketball Camp, the Camp director, or their respective employees, office, agents, representatives,
successors, and/or assignee, for any and all damages which may be sustained or suffered by me in connection
with my association with or participation in, or rising out of my traveling to and returning from said school to be
participated in on the campus of Floyd County High School.
