Our Vision
A healthy and inclusive community
Our Mission
To develop and deliver quality health services that respond to the
needs of our communities, with a particular focus on engaging
those who may not readily access mainstream services
To advocate for a social model of health and address the structural
reasons for health inequity.
SHIP is a five year initiative of Inner South Community Health where residents
and local organisations work together to make Port Melbourne a healthier
place to live. Our four action areas include:

Healthy eating for all
Inclusive and connected neighbourhoods
Affordable and accessible health services
As well as our four action areas, here are a few of the projects that will be
happening in 2015 that you can get involved in:

FoodMate - 8 week program designed by SecondBite to develop the food
These values underpin all our interactions with staff, clients and our

independence of individuals who are at risk of, or experiencing, food insecurity.
The program aims to support people in accessing, preparing and cooking
affordable and healthy food.
1 day a week for 8 weeks and is free. Dates and venue to be advised.

Working with individuals and communities to build their attributes,
capabilities and capacity to manage their lives.

Heart Foundation, we will be organising a local walking group for interested
Port Melbourne residents.
Organised by a volunteer, the walks are free and will be held weekly.
Day and time to be advised.
Queen Victoria Market Bus Trips - a weekly bus trip to the
Queen Victoria Market for residents of public and social housing in Port Melbourne.
Allows local residents to purchase high quality, healthy food at affordable prices.
Thursdays 9:30am-12pm all year.
Gold coin donation.
Waterfront Welcomers - More volunteers are needed for this pilot project
which is currently running at Station Pier in Port Melbourne.
Volunteers act as ambassadors for Port Melbourne and the City of Port Phillip in
welcoming visitors from cruise ships and the Spirit of Tasmania.
They provide information on local activities, attractions, culture, history and heritage
to encourage visitors to spend more time in Port Melbourne and the
City of Port Phillip.

Prahran Strength & Exercise Program
An exercise group for men and women of all ages recovering from injury or with a
chronic health problem that affects their strength, flexibility or balance. The group is
designed to improve health, fitness and co-ordination in a fun, safe environment. You
will be given an individual program and will be encouraged to become independent. A
Physiotherapist or Physical Educator will be present during sessions. ISCH

Family Violence

Warm Water Group
Warm water workout group for adults (18-64) who enjoy exercising in water to improve
health and wellbeing and to maintain their ability to perform everyday tasks.
This weekly group is led by an Occupational Therapist & Physiotherapist and is open to

support to participate in community based physical activity.

In this workshop, you learn ways to simply check and safely care for
your feet. Learn gentle ways to reach your feet using the
Feldenkrais Method ®.

support and care services for frail older people and their carers (65 years and older
and 50 years and older for Aboriginal and Torres Strait Islanders)

The SouthPort Legal Service
The SouthPort Legal Service team provides advice on traffic offences,
criminal matters, family law, tenancy issues, discrimination law, motor
vehicle accidents, and a free Will service. The Legal Service also assists
people to draft their own legal letters and assists with legal documentation.
Home visits and day appointments are also available for the frail elderly and
people with disabilities. This service is available to residents of the Cities of
Port Phillip and Stonnington.

Men’s Responsibility Program
This group is part of the Men’s Responsibility Program. It assists men to stop angry,
aggressive, abusive, controlling and/or violent behaviour in family and relationship
situations, and challenges attitudes associated with this behaviour. Initial one-to-one
assessment interviews (usually two appointments) are required before starting the
group.

Veg Out Gardening Group St Kilda
This is a free service that allows you to discuss and find ways to address smoking that
works for you. Individual sessions are also available separately to group work.
6 week long ‘Fresh Start’ courses run in partnership with Living Positive Victoria,
starting in March 2015. The course offers educational information about smoking
cessation, an opportunity to learn a range of coping strategies and a means to build
confidence to tackle smoking, provided in a non-judgemental and supportive group
environment.

FOOD for THOUGHT—St Kilda Uniting Care Drop In Centre
Our Rainbow Place
This is a fortnightly Thursday 'gathering place' in the Cottage at 18 Mitford St, St Kilda. This is
part of a program for Aboriginal and Torres Strait Islander people, their friends and family, and
welcomes people from any age. There are trips and outings, things to do including an
Indigenous art program running currently. This is a very social place to meet old friends and a
chance to meet new ones over a cuppa and a bite to eat, and speak to staff from Inner South

Wominjeka BBQ
The Wominjeka BBQ is a weekly catch up over lunch for local Indigenous community. It provides an opportunity for the community members to access health and welfare services held

for food, chat and gardening activities. The group is inclusive of older people, people
with disabilities and the Indigenous community. There is an area of raised garden beds

An ISCH Dietitian is available once a month while you wait for lunch to be served. We’re
there to meet you, talk about your diet, record your weight and waist measurements (if you
would like us to) and answer any nutrition questions you might have. No appointment is required. Anyone wanting to see us here will need to meet eligibility criteria for the Uniting Care
Drop-In Centre and also be registered as a client of ISCH by the Dietitian.

Coffee Club
The Coffee Club is a casual, friendly place where consumers of the ISCH Mental
Health Program get together for a cuppa, a chat and board games. This group runs weekly
on Thursdays alternating between Windsor and South Melbourne. Participants need to have
a current or previous ISCH Mental Health Case Manager or Personal Helper and Mentor.
