EXCELLENCE IN OUR HOME AND YOURS
As we celebrated our 40th anniversary last year, we introduced a new logo and name to reflect a broader mission
and commitment to our community. The name, Jewish Senior Services® is an umbrella to our full continuum
of care that is offered to seniors and their families. We are thrilled with the reactions as we now can confidently
help seniors in their homes, as well as here on Jefferson Street. The goal is to be able to answer every family’s
call for assistance with a resounding, “yes – we can help you!” It is not always clear what is needed, but our
dedicated team works with families to determine how best to live life to the fullest in the least restrictive setting.
In 2014, we served more people living in the community than on our Jefferson Street campus through our
many programs and services. Our innovative Senior Choice at Home® program expanded its membership
and continues to help families plan for their long term care needs. Nearly 500 individuals received short term
rehabilitation in our Goldstein Rehabilitation Center. The Friedman Home Care Agency expanded its
reach to hundreds of individuals, with everything from medical care, nursing, and physical therapy with live-in
support or hourly assistance from our dedicated caregivers. The Chaifetz Family Hospice celebrated its second
full year of operations and held a remembrance for those we have cared for. Grasmere by the Sea Adult Day
Program expanded its family support groups, community outreach, and transportation network to serve more
people in the area. Our Institute on Aging continued its promotion of geriatric care management, including
seminars to employee assistance programs. The Center for Elder Abuse Prevention continued its important
work through leadership of the Coalition for Abuse Prevention of the Elderly (CAPE), including its annual
conference. Staff also collaborated with Yale University researchers, participated in the state and federal Elder
Justice Coordinating Council, and provided input to the national Committee on Aging’s nine core principles
for preventing elder abuse.
For those precious seniors who live at The Jewish Home, our flagship nursing home, it was a year filled with
celebrations, birthday parties, family visits, and trips throughout the region. The Journey to Resident-Centered
Care continued to impact all of our lives; perhaps most meaningful were the Neighborhood Actions Teams’
values projects. What an incredible demonstration of the teamwork and commitment of our entire Jewish Home
family to enrich the broader community.

The plans for our new campus, The Harry and Jeanette Weinberg Campus, became very visible this year,
and as you will read we achieved many critical milestones in our development. The purchase of the JCCS land
at 4200 Park Avenue in March, the issuance of tax-exempt bonds in April, the incredible effort to bring the
steel infrastructure to life and all of the supporting elements to build a building were achieved in 2014. We also
announced plans to sell our current property to Sacred Heart University after we move in 2016.
Finally, we are only able to accomplish all of this thanks to the thousands of individual and corporate donors
who continue to support our efforts. We had another successful year with our events that raised over $260,000,
the Annual Campaign that raised over $200,000, and grew programs and services through our Special Purpose
Funds. The campaign for the new campus, The Journey to Home Capital Campaign, has achieved $25 million
towards our $27.5 million goal, which enabled us to build the project as designed.

THE HARRY AND JEANETTE WEINBERG CAMPUS UPDATE
Jewish Senior Services is deeply committed to providing the highest possible quality of life for our Residents. Over the past few years, we
have researched, studied, and evaluated what this means and how it can best be achieved. The result is our vision of a new Home at 4200
Park Avenue — one that reflects warmth, safety, and comfort; upholds each Resident’s right to self-determination; values community; and
maintains outstanding levels of care. Jewish Senior Services is now building this new “Home” that will bring the first “Household Model”
of Assisted Living and nursing home care to Connecticut.
Our plan consists of small groups of Residents living in a household, with amenities they have selected themselves as well as private bedrooms
and bathrooms, an open kitchen, dining area, outdoor terrace, and large gathering room to resemble a traditional family home. The result
is more privacy for Residents; greater freedom in how they live; a calmer, quieter homelike environment; and additional opportunities to
form quality relationships with fellow Residents, employees, and family.
The project includes 280 skilled nursing beds - including 28 dedicated to short term rehabilitation - designed in 14 bedroom houses. The
campus will have 28 Assisted Living Memory Care studios and 18 Assisted Living one-bedroom apartments in the Roz and Les Goldstein
Residences. The campus will also offer our adult day program, outpatient therapy, child care, creative arts center, work activity center, salon,
clinic, and all of our community services offices. Common areas will include our synagogue and multi-purpose room that can accommodate
up to 330 guests, our kosher bistro, and the Chick and Tillie Rosnick Fitness Center, an 18,000 square foot health club, all available to the
Residents, family members, volunteers, employees, and the community-at-large. The newly merged Federation for Jewish Philanthropy of
Upper Fairfield County will house its offices and various programs on the third floor.
THE HARRY AND JEANETTE WEINBERG CAMPUS UPDATE
We have done incredible work with our general contractor, KBE Building Corporation and architects, Perkins Eastman, to bring this building
to life. The infrastructure is in place and we are now working to “fit out” the space to create our new home. Construction is expected to
be completed in early 2016, and move-in will follow after inspections, punch lists, furniture, signage, etc. All of the Residents on Jefferson
Street will make this transition, and we are forming a Waiting List for Assisted Living now.
We are proud of Jewish Senior Services’ tradition of caring and compassion for the past 40+ years and are thrilled with this possibility to
transform the way Skilled Nursing care and Assisted Living are offered to our precious elders.
