APPLICATION FOR GRADUATION
due February 27, 2015—limited to the first 12 applicants
GRADUATION REQUIREMENTS
1. You must be a member of KACH. Go to www.katyhomeschoolers.weebly.com for
membership information. NOTE: Application for membership and payment must be
received before graduation application can be approved.
2. The cost of the KACH graduation is $50 per graduate. You must submit payment
with this application to secure your spot. Please make checks payable to KACH.
3. You must complete this application and send it, with payment, postmarked no later
than February 27, 2015 , to KACH Graduation, c/o Adrienne Danke, 11419 Oak Spring
Dr, Houston, TX 77043.
MANDATORY MEETINGS AND DEADLINES
To participate in the graduation, the following meetings must be attended (with exception of the
first) and all materials submitted by the deadlines listed below. Please enter these into your

The facility we have chosen is limited in the number of guests it can accommodate
according to space and the city fire code.

We can guarantee 10 guests for each graduate. This number includes the parents but not
the graduate. Additional guests will be accommodated on a space-available basis only.

If needed, tickets will be issued to each family according to how many guests you have
requested to invite and space availability. Otherwise, we will be following the honor
system.

RELEASE AND INDEMNITY AGREEMENT
In consideration of being permitted to enter property rented by Katy Association of
Christian Homeschoolers (“KACH”) for the purposes of the KACH Graduation on May 30,
2015, and using certain equipment and/or other items of personal property rented or
owned by KACH,(student)________________________________________________,

guardians, legal representatives, heirs and assigns (hereinafter referred to as "Releasor"), hereby
releases, waives and discharges KACH and its members, leadership, sponsors and Board of
Advisors, and their heirs or assigns (hereinafter referred to as "Releasees"), from all liability to
the Releasor, his/her legal guardians, legal representatives, heirs and assigns, for any and all loss
or damage, and any claim, demand, action, right of action or damages of whatever kind or nature
at law or in equity, resulting therefrom, on account of injury (known or unknown) to Releasor's
person or property, even injury resulting in death of Releasor, whether caused by the negligence
of Releasees or otherwise while the Releasor is on or about any property owned by Releasees for
the purpose above described.
RELEASOR AGREES TO INDEMNIFY RELEASEES AND EACH OF THEM FROM
ANY LOSS, LIABILITY, DAMAGE OR COST THEY MAY INCUR DUE TO THE

Releasor hereby assumes full responsibility for the risk of bodily injury, death or property
damage due to the negligence of Releasor, Releasees or otherwise while in or upon the property
rented by KACH for the purpose of the KACH Graduation 2015. Releasor expressly agrees that
this release, waiver and indemnity agreement is intended to be as broad and inclusive as
permitted by the laws of the State of Texas, and that if any portion thereof is held invalid, it is
agreed that the balance shall, notwithstanding, continue in full legal force and effect.
This Release contains the entire agreement between Releasor and Releasees concerning the
subject matter hereof; and the terms of this Release are contractual and not a mere recital.
Releasor represents that he has carefully read this Release and knows and understands its
contents and signs this Release as his own free act.

GRADUATE BIO
To participate in the KACH graduation ceremony, each graduate must submit a short bio. This
bio will be printed in the graduation program and read by a committee member or graduation
volunteer during the ceremony.
Your graduate bio should be at least 75 words but no more than 125. If you use scripture in your
synopsis, it must be a scripture taken from the 66 canonized books of the Bible. The Graduation
Committee reserves the right to edit the bio where necessary for flow and consistency, or to fit
space available in the program.
Suggested topics for your bio:
 years home schooled
 type of curriculum used
 setting for school (home, outside classes, dual credit at junior college, on-line classes)
 clubs or outside activities
 achievements and/or awards
 honors received
 a brief description of plans after graduation
 anyone you want to thank
 a special bible verse

PHOTOS SUBMISSION
To participate in the KACH graduation ceremony, each graduate must submit photos for the
opening of the ceremony and the diploma presentation. Please understand our deadline is based
on the fact that it takes time to produce a quality presentation with 12 graduates, each with 16 to
18 photos before the graduation.
OPENING CEREMONY PHOTO MONTAGE: The ceremony begins with a photo montage
of pictures from all graduates set to music. Each graduate is required to submit 8 to 10 photos for
the montage. Here are some photo suggestions:
1. Special occasions
2. Service projects
3. Sport or activity photos
4. Group activities with friends
5. Family activities
PICTURES FOR INDIVIDUAL DIPLOMA PRESENTATION: During the presentation of
diplomas a picture presentation for each graduate will be displayed. Each graduate is required to
submit 8 photos for this presentation. The photos should be of the following:
1. A recent picture of the graduate
2. A childhood photo
3. Two action photos
4. Two photos of the graduate with friends
5. A family photo
6. A cap and gown photo or senior photo makes for a great ending
Tips for choosing photos:
1. Avoid pictures that are dark or poorly lighted--they will NOT display well.
2. Pictures taken outside DO display well for the presentation.
3. Pay careful attention to what else or who else is in the background of your pictures.
4. This is a special occasion so please use good taste when choosing pictures.
Guidelines for submitting photos:
1. Photos MUST be in a .jpg format. No exceptions.
2. Please label each file as follows:
LAST NAME-OPENING 1, LAST NAME-OPENING 2, etc. for opening montage
LAST NAME-DIPLOMA 1, LAST NAME-DIPLOMA 2, etc. for diploma presentation
3. Photos should be emailed to Kimberly George at [email protected], or they can be
submitted on a CD. Please use the labeling system above.
4. A CD can be mailed or dropped off to Kimberly George, 3926 Autumn Pines Trail,
Houston, TX 77084 or hand delivered at the cap & gown fitting on March 28.
SUBMISSION DEADLINE:
Photos must be submitted by Friday, March 27, unless you are hand-delivering a CD the next
day at cap & gown fitting. (If you haven’t had a chance to take a cap and gown portrait, we will
allow one photo of the graduate in cap and gown to replace a photo in your collection if it is
submitted by April 24.)
