PLEASE NOTE CLASS SESSION DATES, AS SOME CLASSES BEGIN AND END ON DIFFERENT DATES.
•••Free class available for full time volunteers!•••
Each class must have a parent volunteer. We cannot have a class without one.
The job of the volunteer is to remain in the Student Enrichment class, report attendance to the main office
and to support the instructor and students. Additionally, being a volunteer is fun and allows you to learn
along with your child! A parent volunteer for the full session will have the registration fee waived
for one child.
Lottery: Please note that if your child did not get into Winter session due to class size, they will have
first priority for any class this session. Registration forms must still be completed and returned on
time.

CLASSROOM BEHAVIOR & PICK-UP AGREEMENT
The After School Student Enrichment classes are an opportunity for children to “enrich” themselves. We have found
that if one or more students are disruptive, the entire class is affected. Given this, the following steps will be taken to
ensure all children enjoy their classes.
Student Enrichment classroom behavioral requirements:
1. If your child is disruptive in the Student Enrichment class that he/she is enrolled in, the first step taken will be a phone
call home to caregivers letting them know the situation and then addressing it with the child at home.
2. If your child is disruptive a second time in the Student Enrichment class that he/she is enrolled in, the child’s parent
will be required to attend the next class with them to ensure that the class is able to run smoothly.
3. If the child continues to be disruptive a third time, your child will not be allowed to return or attend the remainder of
the classes for that session or any other Student Enrichment class the remainder of the year.
4. Children must be picked up on time from each class. If pick up is late by more than 5 minutes, the second occurrence
may result in child’s removal from class. No refunds.
1. Student Signature: _______________________________________ Child 1
2. Student Signature: _______________________________________ Child 2
3. Parent Signature: _______________________________________________
Does your child have a serious allergy or medical concern you feel the instructor should know about?
If yes, please provide written details to Student Enrichment to be shared with instructor.
