HOW WE CARE FOR YOU
Contact your insurance company – You are
responsible for obtaining important information,
such as:
• Coverage – Does your plan cover the cost of
treatment at our facility? If not, you must make
payment in full at the time of service. If we do
not participate in your insurance plan, we will
submit your claim for you, but you must still
make payment in full at the time of service. In
either case, for your convenience, we accept
Visa and MasterCard.
• Co-pay/deductible/co-insurance – Does your
plan require a co-pay and/or deductible? If so,
you must pay the co-pay/deductible at the time
of your visit. We are required, as part of our
contract with your insurance carrier, to collect

Medications – You should continue to take all
of your usual prescription medications.
[Note: At each subsequent visit, be sure to tell our nurse if
your medications have changed. If our physicians write
prescriptions for you, you must pick them up here during
office hours (M-F, 8 a.m. – 4:00 p.m.). We cannot mail
prescriptions, refill them early or refill them on weekends.]

From I-84 East or West — Take Exit 43 (West Hartford
Center-Park Road). Merge into the far left lane. At the
end of the exit ramp, turn left onto Park Road. At the
first light, turn right onto Raymond Road. At the second
traffic light, turn left onto Memorial Road. Take the first
left onto Isham Road and drive into the Memorial
Parking Garage. There is designated parking for Pain
Treatment patients on Level 3. Take your parking ticket
with you; we will validate it in our office (up to two
hours). Enter the medical office building on the first
level and take the elevator to the 4th floor, suite 435.

BEFORE YOUR FIRST APPOINTMENT
HOW TO GET HERE.
Provide us with all worker’s compensation
information (if applicable) – If this is a worker’s
compensation case, we must have written or verbal
pre-authorization prior to your visit. It is your
responsibility to provide us in advance with
complete insurance information, including
employer’s name, address and phone number;
insurance company’s name and address; claim
number; and adjuster/case manager’s name and
phone number. If we do not have this information
prior to your visit, we will need to reschedule.

A team effort – You are a critical member of
the multidisciplinary team. Your treatment can
succeed only if you are an active participant in
your care.
If you have any questions or concerns prior to your
appointment, just call us at (860) 696-2840, and
we’ll be happy to help you.

Mutual respect – We pledge to treat you with
compassion and respect. We ask that you, in turn,
treat our physicians and staff with respect.
Arrival – For your first visit, plan to arrive in our
office one hour before your scheduled appointment.
For follow-up visits, you’ll need to arrive 15 minutes
before your appointment.

Comprehensive treatment – We are the only pain
center in Connecticut that addresses all aspects of
pain: medical, physical and psychological.
• Pre-certification/referral – Does your plan
require pre-certification and/or referral by your
primary care provider prior to treatment? It is
your responsibility to work with your primary
care provider’s staff to make sure this is done.
If authorization or referral is not in place when
you come in, we will have to reschedule your
appointment or charge you for the visit.

managing and treating pain. Professionals from
several different disciplines will be involved in
your care.
