We’re Centura Health, and we’re here for your health. www.centura.org Metro Community Provider Network MCPN began in Aurora in 1989, with one clinic and one physician, in response to the limited access to quality health care for underserved and uninsured families in the Denver Metro Area. MCPN’s services are pivotal to providing necessary treatment and care to the medically indigent in our community, reducing unnecessary emergency room visits at community hospitals. 2015 celebrates 26 YEARS OF SERVICE EXCELLENCE in the MCPN community! The organization has grown to 23 health center locations that provide integrated healthcare, case management, and outreach services to the underserved in Adams, Arapahoe, Douglas, Jefferson, and Park Counties. In 2014, MCPN provided healthcare to 42,737 individuals through 292,078 visits/service encounters. MCPN is charged with continued expansion to further meet the growing demand for healthcare and related services in our community. The way MCPN does business as a Community Health Center is shifting due to the Affordable Care Act, as we see a rise in the number of patients who are now enrolled in Medicaid or other health insurance plans. In fact, in 2014 MCPN’s Outreach and Enrollment team assisted patients by filling out 36,363 applications for the health care marketplace! What hasn’t changed is MCPN’s desire to provide every patient health care they can trust, where they have a doctor who listens, quick appointment times, affordable care, and a place where all health care needsphysical, dental, and behavioral health- are met. Every dollar you contribute helps us improve the lives of our patients. If you are unable to participate in the tournament, please consider a generous contribution to MCPN.
