Job Title: CCHAP Pediatric Practice Improvement Coach (PPIC)
Summary
The Colorado Children’s Healthcare Access Program (CCHAP) is a non-profit organization devoted to
ensuring that every child in Colorado receives comprehensive healthcare from a primary care provider a medical home. CCHAP provides a variety of support services for private pediatric and family practices
throughout Colorado and links practices with local agencies and community organizations to enable the
practices to provide quality healthcare for low-income families.
The Pediatric Practice Improvement Coach (PPIC) is responsible for the supervision and provision of care
coordination and resource coordination support services for CCHAP affiliated pediatric and family
practices and clinics throughout the state of Colorado. The PPIC will engage in coaching and managing a
case load of practices and clinics to enable them to provide comprehensive preventive care services that
improve the health of children, and also provide the array of services expected in healthcare reform.
The PPIC will also assist primary care practices to determine how to most efficiently incorporate these
services into their practice. PPIC will assist practices to implement quality improvement projects to
improve care and health outcomes, as well as, develop methods and materials for providing these
services in an engaging, efficient manner.
Qualifications

















2-5 years of experience in a Health Care Setting (preferably primary care)
Bachelor's degree in a healthcare related field required; Master's degree preferred. Basic
understanding of the transition/change process
Excellent customer service skills and the ability to interact effectively with health care
professionals
Excellent verbal and written skills, interpersonal communication, and relationship-building skills
Experience with developing and implementing quality improvement projects and initiatives in a
health care setting
Experience working with healthcare data; data analysis and creating reports
Experience with evaluating, understanding and/or managing the complexities and challenges
experienced by primary care medical practices
Experience with teaching and/or coaching and facilitating meetings and discussions with health
care professionals including medical and behavioral providers
Intermediate to advanced computer software skills especially Microsoft Office applications
Ability to work occasional evening hours.
Understanding of the psychosocial needs and issues of children and families and medical
systems
Understanding of medical systems
Experience with motivational interviewing
Experience in acting as a change agent
Strong organizational and multi-tasking skills
Must be a strong and collaborative team player
Must possess a current driver’s license, and be willing and able to travel throughout Colorado
(up to 50% and occasional overnight), stays up to 20%
Preferred Education Field of Study – Healthcare Management; Social Work; Public Health; and
related fields
Additional Individual Responsibilities











Educate, train and coach implementation strategies for coaching medical homes to improve
measurable health outcomes for children, by providing care coordination to children and
families, implementing best practices and quality improvement strategies, and practice
change/transformation
Develop systems to collect, record, evaluate, interpret and impact practice on a variety of EMR
and state data collection tools, educate practices on the use of data and population health.
Maintain knowledge and skills in quality improvement modalities, clinical best methods to reach
desired health outcomes, practice change/transformation, and relevant changes in health care
policies regarding Medicaid and SCHIP (state and federal).
Participate as an active CCHAP team member by being cross-trained in all CCHAP activities
Participate in CCHAP staff meetings and planning meetings, quality improvement projects, and
recruitment of practices on an organizational level.
Participate in statewide pediatric care coordination training and learning collaboratives.
Travel to practices around the state of Colorado to implement pediatric best practices, quality
improvement and practice change.
Write and maintain guidelines for medical homes on pediatric quality improvement and best
practices for improving care within the primary care practice.
Complete evaluations of pediatric and family practices and clinics to identify strengths and
challenges for barriers to implementing best practices within the practice. Collaborate with the
practice to develop Growth Plan to address the identified barriers and implement the Growth
Plan in conjunction with practice goals.
Document into CCHAP data system (i.e. Salesforce) all work performed with practices and other
contacts.
