Boston cancer specialist
expands treatment options
for women in the Seacoast
A
cancer diagnosis is often overwhelming. Adding the
complexities of cancer surgery at a large metropolitan
hospital makes the process daunting. A new program
eases part of the burden by bringing the cancer specialist to the Seacoast at Wentworth-Douglass Hospital.
As part of Wentworth-Douglass Hospital’s clinical
collaboration with Massachusetts General Hospital,
announced in 2008, John Schorge, MD, joined the
Wentworth-Douglass Hospital OB/GYN physicians to
provide high level surgical expertise for women with
complex cancer diagnoses.
“The patient’s OB/GYN makes a referral and starts
the process. I meet with the patient and if surgery is
appropriate, their GYN and I will do the surgery together. Their GYN doc does the follow-up and discharge.”

including the daVinci Robotic Surgical System to
dramatically reduce the size of surgical incisions and

ven in tough times, with medical problems and medical bills piling up, Newmarket residents Robert and
Rosie Cleary keep on loving each other and making
it work. The Clearys sat on the sofa in front of the
Hospital’s lobby fireplace to tell their story of gratitude
for the people they call “top notch” at WentworthDouglass Hospital. Rosie’s enthusiasm for life makes
a quiet morning turn bright and cheerful. Her expressions are grand and she crosses and uncrosses her

everyone, one by one. Bobby sits quietly, at ease in
a soft flannel shirt, topped by blue suspenders smiling
at Rosie’s superlatives and inserting his own very
gracious words of appreciation for so many people
that have helped him.
After losing his auto parts job and leaving his home
in Farmingdale, Maine, a year and a half ago, Robert
and his wife Rosie, both 57, found an apartment in
Newmarket.With a history of heart problems, he needed
a doctor. A neighbor suggested they visit Dr. Hugh
MacDonald at the then new Great Bay Family Practice
in Newmarket, close to where they lived.
“Dr. MacDonald, well he’s just the best,” Rosie
said. Bobby had some chest pain and Dr. MacDonald
referred him to Dr. Danford and Wentworth-Douglass
Hospital. Since then he’s had two cardiac cath procedures and two colonoscopies. He’s visited Emergency
several times and been an inpatient on 3 North where
the care is “always superb.” Bobby recalls, “I was
scared, scared about what they were going to do to
me and scared about so many things. I didn’t have
any medical coverage. I was going out of my mind
worried about paying the hospital bill too.”

Advance Directives take burden from family and friends

The newly-named Cardiovascular Care Center (formerly
called the Heart Care Center) has purchased two Siemens
Artis Zee angiography systems to perform both cardiac and
angiography procedures. The new equipment provides high
flexibility and fast positioning for patients and physicians.
The system provides a fully digital image chain with highresolution technology as well as a radiation-free positioning
function which allows radiation dose-saving operation for
physicians, staff and patients. The Siemens equipment also
permits the operators full functionality of software applications from the tableside, without leaving the patient.

that family and the health care system follow your wishes on life-sustaining treatments
and your preferences for end-of-life care. It is
important to share your wishes with loved ones
long before critical decisions need to be made.
An Advance Directive is a legal document
stating a person’s preferences for end-of-life
care. They include a Living Will and a Durable
Power of Attorney for Health Care (DPOA-H).
The Living Will takes effect only when there is
no hope of recovery. The DPOA-H takes effect
anytime a physician activates the document
because a person is unable to make decisions.
Advance Directives are not required, but it is
your right to participate in future planning.
A DPOA-H, or health care agent, is a person
assigned by you to voice your written wishes
in the event you are not able to state them.
They are not appointed to make these important decisions for you, rather, to make sure
others are following your wishes. It is important to have a discussion with your health care
agent to assure they feel comfortable carrying
out your wishes prior to appointing them in
your document.
Advance Directives help avoid the stress that
occurs if nothing is in place and immediate
decisions need to be made on your behalf.

a variety of reasons. Participants have the opportunity
to gain access to new research treatments before they
are widely available, frequent exams and close attention that is offered, and the potential to help others
by contributing to medical research.
Clinical trials have been offered at the WDH Seacoast
Cancer Center for 15 years. The Wentworth-Douglass
Physician Corporation offices at Endocrinology &
Diabetes Consultants, Strafford Medical Associates
and Seacoast Arthritis and Osteoporosis Center have
been conducting clinical trials for over 20 years.

Hospital (WDH) is pleased to
announce the opening of its newest
location at the Wentworth-Douglass
Hospital Professional Center at Lee. The
new expansive Rehabilitation Services
Department houses cutting edge technology in a setting of private treatment
rooms and a bright, full-service gymnasium. Services in the 5,000 square foot
facility include physical therapy and
occupational therapy.
WDH senior physical therapist, Chris
Tremblay, PT, and Carmen Lefebvre,
OTC, will coordinate care at the Lee
site. This new, full-service facility can
accommodate patients with nearly any
rehabilitation need.

Wii takes the stress
out of the stress test
“
T
here was no stress in that stress test,” said
Janith Bergeron, of Barrington, a recent
patient in the Cardiovascular Care Center at
Wentworth-Douglass Hospital (WDH).
Janith walked briskly on the treadmill while
watching herself navigate streets in an integrative
video game from Nintendo Wii playing out on
the screen in front of her. Her stress test was far
from the normal walk on a treadmill – staring at a
blank wall. Nurse Practitioner Kathy Wadleigh and
Cardiology Technologist Christina Chase cheered
on Janith as she passed other walkers in the
game, including characters created by each staff
member in the Cardiovascular Care Center.
“It is definitely a distraction and entertaining,”
said Janith. “But the comments from [Christina
and Kathy] were the best – very amusing.”
“Any evaluation of the heart can be stressful
for patients,” said Jonathan Eddinger, MD, Medical
Director of Non-invasive Cardiology at WDH.
“Our goal at WDH is to minimize that psychological
stress while providing a safe and effective test environment. I think the Wii helps facilitate that goal.”
“We are very excited about being able to offer
this as an additional motivational tool for stress
testing at WDH,” said Cindy Harris, Director of
Cardiovascular Services at WDH. WDH was the
first hospital in the area to offer Wii for cardiac
stress tests.

Dover for approvals to change roadways and expand parking to

WDH Auxiliary Donates
$32,000 for Fetal Monitors
W
entworth-Douglass Hospital (WDH) Auxiliary designated their
2008 donation of $32,000 to the Foundation for the purchase
of a fetal monitor and telemetry unit.
The new technology allows the mother to walk around during
labor while still being monitored to insure the baby is tolerating labor,
according to Karen MacDonald, RNC, MS, Maternal Child Director.
“Walking in labor has been shown to shorten the labor length
and improve outcomes with less cesarean deliveries and less pain
medication,” Ms. MacDonald said.
The Birth Center, part of The Women and Children’s Center at
WDH is the fastest growing birth center in the Seacoast area. The
increase in births necessitates an increase in monitors.

entworth-Douglass Hospital & Health
Foundation is celebrating its fifth year of
raising funds for the Hospital. Since the Foundation
began raising funds in 2004 over $4.1 million has
been raised through December 31, 2008 to support
many programs and services at the Hospital. Thank
you for making the first five years so special! The
next five years should be even more exciting and
meaningful.
The Hospital’s commitment to the community
becomes stronger through the partnerships established
with hundreds of donors who believe in our mission
“to help individuals and their families attain their
highest level of health.” The generous and faithful
support of individuals, businesses, employees, physicians and foundations who think of the Hospital each
year enables us to make changes and improvements
not otherwise possible. These gifts help the Hospital to
continue to provide the kind of healthcare services our
community needs, with the quality it demands.

A
fter waiting a year for his mouth
to heal from teeth extractions,
Joe Marchese of Dover, will soon have
teeth to smile at his 50th Anniversary
in May and eat what he wants. The
Stark Avenue resident and his wife
of 50 years are both patients at the
Wentworth-Douglass Community
Dental Center. After removing all Joe’s
teeth, Oral Surgeon Dr. Dan Detolla
put implants where his teeth used to
be in order to hold new dentures.
“We’re very lucky we were able to
get into the Dental Center,” Joe said.
“They took out all my teeth and I had
to wait for the bones to settle. I eat OK,
but I can’t eat a lot of things I like yet. I
should have the plate in about a month.”
Plagued by heart trouble, Joe had
bypass surgery in 1995 and now has a
pacemaker. Mrs. Marchese suffers with
macular degeneration. “My wife yells at
me a lot, for doing too much,” Joe said.
“She’s got trouble with the vision in her
other eye now but she’s on a new medication — we hope will help.”
The Marcheses moved from Hudson,
NH, to be close to their daughter and
son-in-law in Dover. Joe worked for
a glass and aluminum company for 30
years, installing tub enclosures, store
windows, anything with glass. He
started his son in the same business —
a business that took his son’s life in a
tragic work accident.
In spite of their sadness and struggles,
this grandfather of seven, says he’s feeling better and very happy to be in Dover.
C
onnie McCallister is a regular at
Wentworth-Douglass Hospital’s
Pulmonary Rehab class on Tuesdays and
Thursdays. The Care-Van picks her up at
her Rochester home and takes her back
when the class is over. For so many in
this class, just walking any distance is
the hardest thing.
“I don’t know why other hospitals
don’t have this service,” Connie said. “So
many people can’t walk very well — it’s
hard to breathe. If we didn’t have the CareVan we’d have to wait for taxis or have a
friend bring us in.This service is just out of
this world as far as I’m concerned.”

of East Rochester echoed Connie’s sentiments as they boarded the van for the ride
home. Walking slowly with a cane, Ray
said, “It’s a godsend, it’s really a godsend.”
The Care-Van travelled 117,772 miles
in 2008 with 13,912 trips for passengers
just like Connie and Ray. Without the
free transportation system, some patients
would not be able to access the care and

WDH’s two new, state-of-the-art operating rooms are uniquely designed to
meet the needs of advanced minimally invasive surgical techniques. The large
rooms house anesthesia delivery systems and the many pieces of equipment
used in an OR. The equipment booms are fixed in the ceilings to promote optimal access to videos, cameras, and clinical equipment frequently used in videoassisted and robotic procedures. Each OR is fully integrated so that all cameras,
monitors, digital images, and documentation systems are interconnected and
managed from a customized nurses’ station — or “command center” — in each
room. X-rays, CT scans, clinical data and surgical images can be viewed on
strategically placed screens to allow the surgical team to work collaboratively
throughout the procedure. In addition, the newest in surgical lighting is delivered through LED technology offering superior color without heat and shadow.

and help Robert pay for his blood testing supplies. “If I didn’t have this program, my medications would probably

cover everything I take but now I only
pay about $150 a month,” Robert said.
“Lots of people are in a hard place
now. If you need assistance you can
get it,” Rosie added, “people here work
with you.”
The Clearys live quietly, occasionally
go out for breakfast and in good weather they drive to Rye beach to just watch
the ocean. “We don’t have our home
(in Maine) and he got sick, so now
my job is taking care of Bobby. I love
him more now than the day I married
him.” The Clearys will celebrate 25 years

called this process intentional rounding. They enter
the patient’s room, assess the patient and review any
test results. Then the nurse, patient and physician
review the plan, goals for the day, discuss the potential
discharge date and allow time for questions from
patient or families members. Intentional rounding on
our unit has achieved significant exchange of critical
information among the patient, nurse and physician.”
When asked about the value of intentional rounding the Director of the Hospitalist Program, Dr.
Cathleen Ammann, stated, “It has improved the way
we practice medicine in so many ways. Logistically,
it turned out to be much less of a challenge than I
had thought, and has been well worth it.”
Surveys of nursing staff completed annually have
shown tremendous improvement from 78% to 91%
when asked about work relations between doctors and
nurses. Clinical Educator, Jana Otis, RN, BSN, PCCN
said, “Intentional rounding is a way for us all to be on
the same page. It also helps to be part of the discussion
so that I know what is going on, and as a nurse, I can
teach and support the patient and family.”

Spaghetti Squash with
Chunky Tomato-Garlic Sauce
Don’t miss out on the easy and delicious squash
that substitutes wonderfully for pasta.
Ingredients
1 large spaghetti squash – cut in half lengthwise
(See Tip)
1/2 Tbsp olive oil
5 ripe tomatoes – peeled, seeded and chopped
1 clove garlic – crushed
1/4 cup balsamic vinegar
1/4 cup Parmesan cheese
1/2 cup fresh basil leaves – cut into thin strips
Salt & pepper to taste
1. Preheat oven to 350.
2. Wrap the squash halves in foil and place in
baking dish. Bake for 45 minutes, or until tender.
3. Meanwhile, in a large skillet heat oil over
medium heat. Add tomatoes and garlic and cook,
stirring for approximately 20 minutes or until
mixture thickens. Add balsamic vinegar and
season with salt and pepper.
4. When squash is tender, scoop out the seeds and
discard. With a large spoon, scoop the stringy
flesh into a serving dish, top with tomato sauce,
parmesan and basil.
*Time-Saving Tip: Squash may also be steamed
in a microwave-safe bowl! Place 1/2 squash cutside up; add 1/3 cup water and cover. Microwave
on high for 4 minutes, turn bowl 1/4 turn and
cook additional 4 minutes. Repeat until tender.
