you at your scheduled audition. The attached paperwork includes all you need to know about
the production as well as a few forms you need to complete and return to us.
We are asking for 32 measures from a pop/rock musical or song.

Due to the rehearsal schedule we ask that you be available for Rehearsals Monday-Thursday 7-10 PM. Some
Saturdays may be added after 2 PM if needed. Due to summer schedules we try to accommodate traveling,
etc. so please make sure you give your full availability so that we can ensure your success in the production.

CONFLICT INFORMATION

Please list any of the days Monday-Thursday 7-10 PM and Saturdays 2-5 PM that you are unavailable June 8September 27. Please also list if you are going to be late. We will not have rehearsal everyday of every week
due to the production teams schedule, expected vacations of cast, other performances so please be as
accurate as possible. The production team is expecting a large amount of absences that is why we are starting
earlier. If you do not mark dates on this form you will be expected to be at ALL scheduled rehearsals.
Rehearsals starting Monday, June 8, 2015 7-10 PM. Rehearsals are MANDATORY for the Cast Called.
Due to the vocal demand this show puts on performers the creative team is considering COVERING (an
understudy from the ensemble). Covers will be asked to attend music rehearsals and blocking to take notes
and learn the scenes and will fill in at all the characters “scheduled absences”. Covers will receive at least one
performance. Please be advised that you will not have to be at all rehearsals a rehearsal schedule will be
completed and e-mailed to you by June 10, 2015.

MEDIA RELEASE
I, the undersigned, hereby authorize MCTP to photograph me, take motion pictures of me, take video footage
of me, and/or make electronic sound recordings of me (herein referred to as photographic or electronic
reproductions).

but not limited to educational and other public media as may be deemed appropriate by MCTP (I understand

WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT
1. IN CONSIDERATION for participation in Metropolitan Community Theatre Project’s (MCTP) production and
other valuable consideration, I hereby RELEASE, WAIVE, DISCHARGE AND CONVENANT NOT TO SUE MPAA/
MCTP, its Board of Directors, officers, servants, agents, and employees (hereinafter referred to as RELEASEES)
from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to
any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me,
WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES, or otherwise, while participating in such activity,
or while in, on or upon the premises where the activity is being conducted or in transportation to and from
said premises.
2. To the best of my knowledge, I can fully participate in this activity. I am fully aware of risks and hazards
connected with the activity, including but not limited to the risks as noted herein, and I hereby elect to
voluntarily participate in said activity, and to enter the above? Named premises and engage in such activity
knowing that the activity may be hazardous to me and my property. I give my permission for the use of my
name and likeness to be used for informational and publicity purposes. I VOLUNTARILY ASSUME FULL
RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that
may be sustained by me, or any loss or damage to property owned by me, as a result of being engaged in such
an activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES or otherwise.
3. I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS THE RELEASEES from any loss, liability,
damage or costs, including court costs and attorney's fees, that may incur due to my participation in said
activity, WHETHER CAUSED BY NEGLIGENCE OF RELEASEES or otherwise.
4. It is my express intent that this Release and Hold Harmless Agreement shall bind the members of my family
and spouse (if any), if I am alive, and my heirs, assigns and personal representative, if I am not alive, shall be
deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above named RELEASEES. I
hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in
accordance with the laws of the States of Washington and Oregon.
5. I UNDERSTAND THAT MPAA/MCTP WILL NOT BE RESPONSIBLE FOR ANY MEDICAL COSTS ASSOCIATED WITH
AN INJURY I MAY SUSTAIN.
6. I further agree to become familiar with the rules and regulations of MCTP concerning conduct and not to
violate said rules of any directive or instruction made by the person or persons in charge of said activity and
that I will further assume the complete risk of any activity done in violation of any rule or directive or
instruction.
7. I also understand that I should and am urged by MPAA/MCTP to obtain adequate health and accident
insurance to cover any personal injury to myself which may be sustained during the activity or the
transportation to and from said activity. IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I
have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily
as my own free act and deed; no oral representations, statements or inducements, apart from the foregoing
written agreement, have been made; I am at least eighteen (18) years of age and fully competent; and I
execute this Release for full, adequate and complete consideration fully intending to be bound by same.

CAST CONTRACT
As a member of the cast of MCTP’s RENT, I agree to adhere to the following rules and requirements:
1. I will arrive on time to all of my scheduled rehearsals, performances, and cast meetings. If I have a
conflict with the rehearsal schedule, I will make prior arrangements with the Production Staff.
2. I will be prepared and professional at all rehearsals and will bring my script, score, and a writing utensil
to all blocking or singing rehearsals.
3. I will respect my fellow performers and the staff of RENT.
4. I will leave the rehearsal space exactly as I found it. The cast is responsible for helping reset furniture
and ensuring that the studio is kept clean and orderly.
5. I will not attend any rehearsals, performances, cast meetings, or strike under the influence of drugs or
alcohol, if I do, I will be immediately excused from my role.
6. I will do my best to attend poster hanging sessions required for all cast members. I will participate in
the selling of tickets and Ads to ensure the success of our production.
7. During build, load in, performances, and strike, I will respond quickly and efficiently to all requests
made by the technical staff, designers, build crew, and run crew to ensure my own safety and the
safety of others.
8. I will accept the costume and make-up designs for my character without complaint. If I do have an
issue with my costume or make-up, I will discuss the matter privately with the Creative Director. I will
not change my hairstyle without prior approval from the Director, Stage Manager, or Costume
Designer.
9. I grant MCTP the right to publish, in perpetuity, media containing my person, including photographs,
videos, and audio recordings. I waive the right to inspect or approve versions of media containing my
person when used for official publication or business.
10. I will uphold the image of MCTP by using Social media (Facebook, twitter and other) avenues to
positively promote myself and MCTP and their staff.
11. If I take issue with another cast member or staff member, I will bring the issue to the Director
accordingly.
12. I am aware that each cast member will be required to pay a $20 insurance Fee, This will cover you for
our season.
13. I am aware that due to the emotional and draining vocal demands of the show, COVERS may be cast
based on the needs of the production. If so a rehearsal schedule will be done to accommodate both
actors.
14. For the better of the production and to ensure the quality of my own experience, I will do my very best
at all rehearsals, performances, and Cast meetings.
The violation of any rule is determined by the Creative Director. Consequences range from a warning to
expulsion from the production and are likewise at the discretion of the Creative Director.
