GUIDELINES FOR APPLICATION OF A NEW PASSPORT

Completing the Form

The Application Form comes with clear and detailed instructions on the front page, which should
be carefully read and complied with. The applicant must provide precise information, as any error
will delay the application.

Complete all sections of the Form, which are relevant to you in CAPITAL LETTERS with BLACK
ballpoint pen, well within the white-boxed areas.

Please note that:

Consent at Paragraph (4A) is required in case applicant is a minor (under 18 years)
Is to be signed by both parents (where applicable), in the presence of a Consular Officer of
the Embassy of Mauritius / Honorary Consul of Mauritius in the country of residence
Section (4b) to be attested in the presence of an Embassy Staff/Honorary Consul of
Mauritius
If aged 18 years & above the applicant need to sign the Application Form at Paragraph 7 & 8
Both parents to affix their signatures at Section 8 in case applicant is a minor (under 18)

Applicants from US, Canada and surrounding Countries
A bank cheque to be drawn in favor of the Embassy of Mauritius, Washington, DC. This Embassy will not
entertain cash payments. A prepaid label or an account number by a courier service (FEDEX, UPS or DHL) is
required so that the original passport and other original documents may be sent back to the applicant.

(a) Application Form (PF 136) duly filled-in
(b) Two (2) recent identical passport-sized photographs as per specifications at Note 2
(c) Previous passport (original + photocopies of data pages)
(d) National Identity Card [NIC] (original + photocopy)
(e) Copy of residency documents (Permanent resident card, Certificate of Citizenship/
Visa or Certificate of Naturalization, whichever is applicable.

(g) Copies of Certificate of Foreign Citizenship/Residency permits of parents)
Both parents need to sign Section 4(A). The consent at Section 4(B) can be given at
the Mauritian Embassy/Honorary Consul/Police Station/Judiciary/Court of Law/Notary
