Requirements for ALL tent vendors • • • 10 x 10 WHITE POP-UP TENTS ONLY. Tents may be rented from MILLWOOD ‘OHANA PRODUCTIONS for a fee of $150.00 which includes set up and take down. Fire Extinguisher at each vendor booth/truck All trash must be removed from event site. Vendors are forbidden from using City trash receptacles for trash disposal. Non-compliance will result in an additional charge or future event participation. See Event coordinator for allowed trash disposal locations. ALL VENDORS participating in this event must have General Liability Insurance with limits of at least $1,000,000 occurrence and $1,000,000 aggregate. All vendors must issue a certificate of insurance naming the following as Certificate Holder/ Additional Insured for the Millwood Ohana Productions: 1. Go Holoholo, Inc. This is a requirement of our general liability policy. *MILLWOOD OHANA PRODUCTIONS PROVIDED INSURANCE - If you do not have a General Liability Insurance policy MILLWOOD OHANA PRODUCTIONS can provide coverage per event for a fee of $75.00. DBA Millwood Ohana Productions, its affiliates, subsidiaries, officers, directors, employees, agents, consultant, servants, attorneys, insurers, and persons employed or engaged by them, as well as their respective heirs, personal representatives, successors and assigns, hereinafter collectively called “ The Great Hawaiian Food Truck Festival” from any and all liability to me, my spouse, legal representatives, heirs, personal representatives, successors and assigns, for any and all loss, damage or injury, and any claim or damages resulting thereof, on account of injury to my person or property, even injury resulting in my death, whether caused by the negligence of The Great Hawaiian Food truck Festival or otherwise, while I am conducting such Activity. I agree to indemnify Millwood Ohana Productions from any loss, liability, damage, or cost (including reasonable attorneys’ fees) incurred due to my participation in and presence at the activity, whether caused by the negligence of Millwood Ohana Productions or otherwise. I hereby assume full responsibility for the risk of bodily injury, death or property damage due to the negligence of Millwood Ohana Productions or otherwise, while participating in or being present at the Activity. I agree to never initiate any action or suit at law or in equity against Millwood Ohana Production pertaining to my participation in or being at the Activity. I expressly agree that this Release, Indemnity Agreement and Covenant Not to Sue is intended to be as broad and inclusive as permitted by the laws of the State of Hawai’i Revision 032415 and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. I am executing this agreement for my spouse, my legal representative, heirs, personal representatives, successors, assigns, and myself.
