RACE RELEASE: I know that participation in races are potentially hazardous activities. I should not enter and participate in this
race unless I am medically able and properly trained. I agree to abide by any decision of a race official relative to my ability
to safely complete the race. I assume all risks associated with participation including but not limited to falls, contact with other
participants, the effects of weather, including high heat and/or humidity, the conditions of the road and traffic on the course, all
such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration
of your accepting my entry, I, for myself and anyone entitled to act on my behalf, waive and release the North Central Arkansas

Miracle Makers 5K

If you do not pre-register, you may register on the day of the race at 8:30 a.m.

Pre-registration is open until Friday, March 27, 2015.

Pre-registration price is $25.00. Day of registration is $30.00

Registration begins at 8:30 April 4th at the Starting line.

Divisions will be Runner: Male, Runner: Female, Walker: Male, Walker: Female

Age groups: 1-9, 10-14, 15-19, 20-29, 30-39, 40-49, 50-59, 60-69, 70 and over

You may mail your entry form to Miracle Makers, 720 East South Avenue, Harrison, AR 72601 Please
make checks payable to NCA

Each pre-registered participant must sign for their own t-shirt and bib number in person. Your signature
is required.

All participants and friends/family members must park vehicles in the Junior High Parking Lot. No
parking in the Soccer Field parking lot due to the
location of the finish line.

The top three finishers (Male and Female) in each
age category will receive medals. A Corporate
Participation plaque will be given to the team with
the most participants.

Please DO NOT remove your race number from
the FRONT of your shirt at any time, even after
you finish the race. We need these for winning
photos.

Walkers may not run. Any registered walker that
jogs or runs any portion of the race will be
disqualified from the walking competition.

Runners may walk.

Please stay to the right of the trail to allow
participants to pass on the left. Walkers, please
yield to runners.

Corporate Team Information
Must appoint a Team Captain to be the contact
person
 Must have a minimum of 15 participants
 Must have an entry for each participant


Please do not bring your pets to the event.

