2015 Season Ticket Group Order Form Instructions Ticket Order Forms MUST be completed on the attached forms & submitted by email before Contestant’s Day, April 11th. Order form ONLY needs to be printed and turned in with payment on Contestant’s Day. A Group Order is any order with 10 or more Season Tickets to the Miss Oklahoma Pageant and/or Miss Oklahoma’s Outstanding Teen Pageant. Orders submitted on a Group Order Form will be processed, seated and mailed together. Families may benefit by consolidating their order with their Local Director. All Group Orders must be emailed prior to April 11th to [email protected] Payment must be turned in on Contestant’s Day - April 11th All Group Orders will be mailed by May 10th The Top 3 Salute Ad Sales Winners (& their groups) will be seated first. All others will be seated by random drawing. Group Form Instructions: Checks: Collect checks from each ticket purchaser in your group, deposit in your bank account and write one check for the total amount to Miss Oklahoma. Submit only one check with each group order! Charges: Miss Oklahoma prefers one Credit Card be used per group order but recognizes that may not be feasible in all cases. Complete a Credit Card voucher form for each card used and bring with you to Contestant’s Day April 11. If your will have your card present, no need to print form. Cards will be charged on Contestant’s Day. (Visa, Mastercard & Discover only) Complete the “Group Order Form. The $15 shipping/handling fee MUST be included or order will be returned. Complete the form “Program Book Listing” with names of each Season Ticket Purchaser & Level purchased. Email Group Order Form to [email protected] Email Program Listings to [email protected] Print your Group Order form (1 page) & Credit Card Forms (if needed). Attach check (if applicable) & bring to Contestant’s Day. Purchase Orders: Email order as instructed above, print and bring to Contestant’s Day with Purchase Order attached. Non- Group Orders and Single Night Tickets: Call the Mabee Center Box Office (918-495-6000) or visit www.missoklahoma.org to place your order after May 1st. Orders will be processed and mailed as received. Requests to be seated near a group may be accommodated but cannot be guaranteed.
