This is your chance to BRING A FRIEND to gymnastics! During the week
of May 18th-23rd, current students of More Than Gymnastics are invited
to bring a friend who is not currently registered for program classes, with
them to participate. Your gymnasts’ friend must accompany them to
their regular program day and time. This is a great way for your gymnast
to show his/her friend how they spend their time at MTG!

If your friend enrolls in the summer session, you will receive $10 off a future tuition session.
**All friend participants must bring this form or sign up online to be admitted- NO EXCEPTIONS. **

I, the Guardian, and on behalf of the Participant, agree to follow the rules set by More Than Gymnastics (“MTG”). I understand that failure to follow such
rules may result in suspension from participation.
I understand that the above listed Participant is using the facilities of MTG at my/his/her own risk. I understand that gymnastics, like any other activity
involving motion, rotation, and height, involves a risk of injury, including landing improperly on your head or neck. Injuries can range from cuts and scrapes,
broken bones, paralysis and even death. These risks may be caused by the Participant’s actions or inactions, or the actions or inactions of others, or the
conditions in which the activities take place. Although the incidence of injury may be small, no coach, no equipment, and no procedure can completely
eliminate these risks. I fully accept and assume all such risks as a result of Participant’s participation of gymnastics at MTG.

CONSENT: I, the undersigned Guardian do hereby grant authority to the staff of MTG to render judgment concerning medical assistance or hospital care in
the event of an accident or illness during my absence.
