NS 062 Issue 2 Rev 2 NORTH STAR SHIPPING (ABERDEEN) LTD. LIFTING EQUIPMENT REGISTER NS 062 Issue 2 Rev 2 Purpose: The aim of this register is to enable the Master of each North Star vessel to accurately record the details of the lifting equipment on board his vessel. Scope: The register is intended to cover all certified lifting and Life Saving Appliance (LSA) equipment on board the vessel. The pages should be completed firstly for the existing equipment on board and altered / updated as equipment is renewed, replaced or retested. Responsibilities: Generally speaking, the Mate will be nominated as the person responsible for conducting visual examinations of equipment. VISUAL INSPECTIONS OF ALL EQUIPMENT MUST BE CONDUCTED PRIOR TO USE. All fixed lifting equipment MUST be inspected before use and maintained in accordance with the Planned Maintenance Schedule for that vessel. Guidelines: When the sheets have been completed, as per the specimen sheets, the document should be kept updated as and when changes are made to any of the equipment recorded therein. This should also form part of the regular hand-over procedure where the oncoming responsible person is made aware of any changes or impending changes to the equipment which may affect the document. Should any piece of the equipment become damaged, this piece of equipment should be condemned by the competent person, removed from use (Quarantined) and an entry made in the register. Fall-wire change-out is carried out on an annual basis, so far as is practicable, on North Star vessels, and these change-outs must also be recorded in the register. Basically, any and all changes / renewals, alterations, inspections and re-certifications must be recorded in the register. This document will be asked for by surveyors, auditors (both external and internal) as objective evidence that the vessel has a positive attitude to safety and positive attitude towards the company’s Integrated Management System. The Master has the responsibility for maintaining an up to date register of lifting equipment onboard. The Master is also responsible for the correct use and stowage of the equipment. Any deficiencies noted during the voyage requiring independent inspection, re-certification or re-test should be notified to the Operations Dept & Rigging Dept in a timely fashion in order that the required action can take place without causing undue delay to the vessel. NS 062 Issue 2 Rev 2 All lifting operations involving vessel’s crane(s) shall be planned and executed in accordance with LOLER, with a suitably constructed Lifting Plan in place. These lifting plans should be used on a regular basis. A blank Lifting Plan example can be found on Page 9 of this guide. Additionally, all lifting operations should have a minimum of three persons: Crane operator Banks-man Load operator Powered appliances should always have a person at the controls while it is in operation; it should NEVER be left to run with a control secured in the ON position. The person operating any lifting appliance should have no other duties which may interfere with their primary task. They should be in a proper and protected position, facing controls and so far as is practicable, with a clear view of the whole operation. Signallers (Banks-man) should be in a position that is: Safe In plain view of the person to whom they are signalling unless an effective system of radio or other contact is in use. The banks-man is NOT to touch or interfere with the load at any time. All signallers (banks-man) should be instructed in and follow a clear code of signals, agreed in advance and understood by all concerned in the operation. NS 062 Issue 2 Rev 2 When slinging loads, the following constraints apply: All slings must be registered in the Lifting Equipment record. A sling must not be used to lift a weight greater than that indicated on the certification label. Therefore, the weight of the load must be determined prior to slinging. Use long slings whenever possible to reduce the angle of the sling. It is important to remember that the lifting capacity of the sling is affected by the angle of the sling legs. Always use two or more legs of a wire rope sling with hook and spliced eye. A single wire can untwist allowing the splice to open and slip. Do not rest a load on a sling Slings must not be knotted. Slings must not be dragged either loaded or unloaded across the deck. Ensure the crane hook is placed centrally over the load to prevent swinging when the load is raised. Stow slings in their designated stowage; don’t leave them lying about! Loads should not be lifted over a person or any access way and personnel should refrain from passing under a load which is being lifted. Spliced ropes may not be used as lifting slings unless they have an appropriate test certificate. This applies to all slings, including those on rescue baskets and gangways. Further guidance to lifting operations can be found in the Code of Safe Working Practices for Merchant Seaman, chapter 21 Inspection Procedures Visual inspections MUST be made of all equipment PRIOR TO USE. At intervals not exceeding 6 months, Craig Group rigging department will visit every vessel to conduct equipment inspections of ALL LIFTING EQUIPMENT. All equipment will be tested ashore at intervals not exceeding 5 years for re-issue of the equipment test certificate. Planned Maintenance should notify ship’s staff that due dates are approaching. However...THE VESSEL IS RESPONSIBLE FOR ENSURING THAT NSS TECHNICAL DEPT IS NOTIFIED THAT EQUIPMENT IS DUE A FIVE YEARLY TEST. Every piece of equipment should have a dedicated plastic wallet containing a copy of the test certificate, inspection record sheet and any other relevant information. Examples of equipment record sheets can be found on page 10. FIXED EQUIPMENT i.e. PAD EYES, LIFT / RUNWAY BEAMS etc CAN BE LISTED TOGETHER ON THE FIXED / PERMANENT LIFTING EQUIPMENT SHEET (page 13). Should a piece of equipment leave the vessel, the document wallet must accompany it. Conversely, new equipment arriving on board MUST be accompanied by its test certificate. NS 062 Issue 2 Rev 2 On inspection one of three results will occur. 1) Equipment Passes Inspection – All equipment that has passed inspection will have to be colour coded to the colour specific for that time period. The colour will be white, grey or honeysuckle. The relevant time periods are detailed on the inspection record sheet. Additionally, the lifting and hoisting state board must also be updated with the new colour. The Mate must then ensure that the inspection record sheet in the register is updated and signed. Craig Group rigging department will then issue a report to the ship and to the QHSE department at North Star Shipping in Aberdeen. 2) Equipment Fails Inspection (Repairable) – The equipment must be quarantined IMMEDIATELY. The Rigging department will remove the item from the vessel along with its document wallet, to the quarantine area at their store. They will then arrange for collection so the equipment can be repaired. NS 062 Issue 2 Rev 2 Once repaired, the item will be stowed by the Rigging Dept with its relevant test certificate. Craig Group rigging department will then issue a report to the ship and to the QHSE department at North Star Shipping in Aberdeen. At no time should any equipment be used without documentation being available onboard. Equipment Fails Inspection (Scrap) - The equipment must be quarantined IMMEDIATELY. The Rigging department will remove the item from the vessel with its document wallet, to the quarantine area at their store. That piece of equipment will then be destroyed. If a piece of equipment fails inspection during monthly checks, it must be quarantined immediately. All vessels are to nominate a quarantine area on board the vessel and notify the QHSE department. The QHSE department must be notified of any equipment inspection failures. When the vessel enters port, that equipment will be removed by the Rigging department. Craig Group rigging department will then issue a report to the ship and to the QHSE department at North Star Shipping in Aberdeen. New Equipment All new equipment will be stored at Craig Group Rigging Dept’s Store before issue. On arrival at the ship, the equipment MUST have a copy of a valid test certificate with it. Do not accept any equipment that does not have a test certificate.
