your name, you do not have your 40 community hours completed. A listing of community involvement
opportunities can be found at www.communityreach.ca. Remember to check the bulletin board outside of
Guidance and listen to the morning announcements.

Attached is a list of awards and scholarships which we anticipate will be presented at this year’s graduation. The name of the award/scholarship, a
brief outline of the criteria required to qualify and the monetary value of each award is listed. You can only apply for the awards marked with
“**Apply”. You must complete all sections on the application form. Please note that some of the awards require that you apply directly to the donor,
and that either an essay or an application form be submitted by a specific date. Contact the office for specific due dates.
Please note that some of the scholarships are awarded with a Conditional Letter. This means that a tuition receipt from a post-secondary institution
confirming your registration must be presented to the office before you can be issued the value of the scholarship. If you wish to apply for any of
these or want more information about any of the awards and scholarships, please, see Mr. Dallos in the Guidance Office. Application form is

The aim is to help compassionate students entering the nursing field through financial assistance. The
grant will be based on a balance of marks and need with the intent of the student to remain in Huronia
after graduation. The applicant should demonstrate commitment for community service.

developmental services.

Students eligible to attend graduation will be asked to nominate a fellow graduate to be Valedictorian.

MSS Graduation will be held on Thursday, June 25, 2015 @ 7:00 pm
A grad gown is required to be able to participate in the graduation ceremony. For the
fee of $15.00 you will receive a grad gown, stole and cap, all of which will be yours to
keep as a memento of your graduation.
Please return this completed form to the Guidance Office by Friday May 8th.
You must pre-book and pay for your cap & gown. Extra gowns are not available the
evening of graduation. Your gown is not ordered until payment is received online.
