At some point, we’ve all tried to eat healthier, lose weight, quit smoking and/or start an exercise program. We begin with the best of intentions, but barriers, such as lack of motivation and support, can sometimes make things rocky. Fortunately, you don’t have to do it alone—wellness coaches are just a phone call away. MAKING HEALTHY CHANGES HAS ITS REWARDS We realize this isn’t the size of your favorite sports hero’s bonus, but we do want to recognize and reward your progress toward better managing and improving your health. Complete specific activities, and you can earn monetary contributions to your health reimbursement account (HRA). You can earn up to $600 in HRA contributions—and your spouse (if applicable) can earn up to $600. That’s $1,200 to pay for your health care expenses that are not already covered by the plan at 100 percent! Health assessment Personalized Health Action Take a 15-minute timeout to complete your confidential health assessment. You’ll earn $200 in your HRA if you submit it between January 1 and March 31, 2015. Otherwise you will earn $100 if you complete it between April 1 and October 31, 2015. Completion of the health assessment sets the stage for you to earn additional dollars in your HRA. Call health connect to work one-on-one with a nurse or wellness coach to develop a Personalized Health Action (PHA) plan based on your unique needs, care opportunities and preferences. Your PHA plan can include key health-related activities such as losing weight, quitting smoking, maintaining a healthy pregnancy, managing diabetes or setting up a checkup with your doctor. As you work with your personal nurse or coach to achieve the goals that have been built into your PHA plan, he/she will inform you when you have met the requirements to receive your $150. Preventive health screening or annual physical exam One of the best ways to get—and stay—healthy is to get regular preventive care. To earn $100 in your HRA, complete one of the following: annual physical exam, mammogram screening, prostate cancer screening, colorectal screening, biometric screening/lab panel or cervical cancer screening. Online wellness program In between checking the scores from last night’s game and browsing the web for your next outdoor adventure, squeeze in an online wellness program. You can earn $75 per program in your HRA by completing trackers, articles, slideshows, and videos. Please note: Max of three programs per year ($225).
