The 2015 NYSESLAT will have 3 answer sheets: Page 1-2, Page 3-4 and Page 5-6. 2. NERIC requires that all answer sheets, Class Rosters, and completed N24-2 forms must be returned to NERIC after the tests have been scored. 3. Return Test Count Sheet: this form must be completed for every exam every time answer documents are sent to NERIC or a Regional Scoring Site. 4. New Scoring Mode Code: #6 Scored by a private contractor (not a BOCES). . Enclosed, please find the preprinted answer sheets for your upcoming testing. Please note there are separate answer sheets by grade level: Grades K Three Sheets Form # TS8212, 8213, & 8214 Grades 1-2 Three Sheets Form # TS8215, 8216, & 8217 Grades 3-4 Three Sheets Form # TS8218, 8219, & 8220 Grades 5-6 Three Sheets Form # TS8221, 8222, & 8223 Grades 7-8 Three Sheets Form # TS8224, 8225, & 8226 Grades 9-12 Three Sheets Form # TS8227, 8228, & 8229 Review the answer documents to make sure you have the correct sheets for your district. Please DO NOT write in, place labels on, or make any marks in the area at the top of the answer sheet where NERIC has preprinted the District Number, Student ID, CD, and School Number. 3 0 A Class Roster will accompany the answer sheets. This roster is a list of your students and their student numbers. If any demographic information is incorrect on the Answer Sheet, please write the correct information in the shaded area only. On the Class Roster, please cross off any incorrect information and write the correct information. Send a copy of the Answer Sheet Roster to your district CIO. Extra Blank Answer Sheets are NOT included in your shipment of preprinted answer sheets. If you are missing a NYSESLAT answer sheet for a student you must contact NERIC immediately. We request you provide NERIC with limited demographic information via e-mail for any student that did not receive a preprinted NYSESLAT answer sheet. You must inform your district CIO of this issue; your CIO must update the student’s data in the Data Warehouse. For each student whose preprinted answer sheet contains incomplete or incorrect information (i.e. Date of Birth, Local ID Number, Grade, etc.), include that student on an N24-2 form. Students in the same school may be recorded on the same N24-2 form, regardless of grade or homeroom. The data you provide on the N24-2 form is used to complete scoring of the test alone. Send a copy of all completed N24-2 forms and Class Rosters to you district CIO. If you were missing NYSESLAT answer sheets and had to complete an Excel spreadsheet for NERIC, make a copy of that spreadsheet for your district CIO. These documents must be sent to your district CIO to insure that these students’ data are in the data warehouse prior to NERIC scanning the answer sheets. If these students have not been added to the Data Warehouse in Demographics, Enrollment, and Program Services, and pushed to Level 1, the assessment data cannot be loaded. These data in the Data Warehouse will be used by NYSED for all statewide reports. IMMEDIATELY following the last day of the testing period, 5/15/15, or ASAP; give a copy of the following, if applicable, to your district CIO: Preprinted Class Roster with notes, and completed N24-2 Forms. **Be sure to make a copy for your records.** NOTE: There are no test booklet labels included in this shipment. MetriTech will provide you with the labels for the NYSESLAT Test Booklets. After the students have completed the Session 1-Listening/Reading, Session 2-Listening/Reading, Session 3Listening/Reading place the answer sheets in secure storage. When Speaking/Writing Sessions of the NYSESLAT are completed, retrieve the Session 1, 2, and 3 answer sheets from secure storage and match each student's answer sheets with the student's Writing Booklet. Place the Writing Booklet face down (you should see the Student Label) and slip the answer sheets into the booklet with the tracking marks (the series of parallel bars down the side of the page) inside the booklet. DO NOT FOLD the answer sheets. The answer sheets are ready for the scoring of the Writing and Speaking sessions. Reminder, each grade has three answer sheets per student. Please keep each student’s answer sheets together. After scoring is completed please bundle the completed answer documents with paper tape. Place the District Return Test Count Sheet on top of the bundled answer documents. Don’t forget to include answer sheets from your out-of-district placement students. All NYSESLAT answer sheets MUST be returned to NERIC. Place in a sturdy box or large reinforced manila envelope and ship to the address below. Per NYSED, answer sheets must arrive at NERIC no later than 4:00 PM, Thursday, May 28, 2015. DO NOT include Test Booklets. Each session has its own Absent bubble. Fill in the appropriate bubble if the student was absent for only that session (i.e. Listening, Reading, Speaking). Refused to take entire test bubble on the sheet should be filled in for students who refuse to take the entire exam. These students will be considered to have "no valid test score" and will be counted as not tested in verification reports and for accountability calculations. Administrative error, no score bubble should be filled in for a student who is unable to take the exam due to an administrative problem or for students who take the test and it is determined after-the-fact that an administrative error has taken place. You must inform SED and NERIC of any misadministration (Please see Coding of Invalid Tests in the SAM). Not enrolled at time of test bubble must be filled in for students who: 1. Have moved out of the district prior to test administration. Be sure to inform your CIO. 2. EXCEPTIONS – a) If you receive a preprinted answer sheet for a student, the level of test is correct (such as 1-2) but the student’s grade is not correct (example: student is in grade 2 but grade 1 is printed on the sheet), DO NOT bubble “Not Enrolled at Time of Test.” On the answer sheet in the shaded area draw a line through the wrong grade, write the correct grade, and on the roster report write “in grade #.” This sheet can be used to administer the test. Fill out the N24-2 form for this student. Be sure to make a copy of the N24-2 form for your CIO. b) If you receive a preprinted answer sheet for a student, the student’s grade and level of test is not correct (example: received Grades 1-2 level answer sheets but the student is in grade 5), DO NOT bubble “Not Enrolled at Time of Test.” On the answer sheet in the shaded area draw a line through the wrong grade, write the correct grade, and on the roster report write “in grade #.” DO NOT USE THIS ANSWER SHEET – Contact NERIC immediately via e-mail with the student name and grade level. NERIC will ship the correct level answer sheet to you. Fill out the N24-2 form for this student. The preprinted answer sheet must be returned to NERIC. Be sure to make a copy of the N24-2 form for your CIO. c) If you receive a preprinted answer sheet for a student who is not taking the NYSESLAT, on the answer sheet in the shaded area write “Not taking NYSESLAT” and write the same on the roster report on the line provided. Be sure to inform your CIO that the student received a NYSESLAT answer sheet but did not take the test. Rev. 3/26/15 WA Page 4 of 5 Medically excused bubble should be filled in for a student: who is incapacitated by illness or injury during the test administration and make-up periods and have on file documentation from a medical practitioner that the student is too incapacitated to be tested at the school, at home, or in a medical setting. Medically Excused students, are considered to have no valid test score, and must be reported with a final score of 999 and a standard met code of 93. These students are excluded from the numerator and the denominator of the participation and performance accountability calculations. • Test Administrators should review completed answer sheets to make sure bubbles are filled in correctly, that all erasures are complete and that there are no stray marks on the answer sheet. Never use liquid paper or correction fluid on the answer documents. • For each student whose preprinted answer sheet contains incomplete or incorrect information (i.e. Name, Date of Birth, etc.), include that student on an N24-2 form. Be sure to make a copy of the N24-2 form for your CIO. • Note 1: Scoring Spring 2015 on page 4 is REQUIRED! DO NOT LEAVE BLANK. Note: There is a new Scoring Model Code; #6 Scored by a private contractor (not a BOCES). • Note 2: In order for a student to receive a NYSESLAT scale score and performance level, a raw score must be calculated for each session of the test; i.e. Listening, Speaking, Reading, and Writing. • If you have a question or are unsure about any of these instructions, contact Bill Adam or Gail Newcomb at 518-862-5310 / 5355 or [email protected]. You may send your NYSESLAT answer sheets to NERIC any time prior to this date. But, per NYSED, NYSESLAT answer sheets must arrive at NERIC no later than 4:00 PM, Thursday, May 28, 2015.
