Fitness Reward
NetCare will reward you just for Working Out!
Sign up at any of our participating fitness centers:
NetCare will reimburse members or pay the fitness facility up to $180 per Contract Period if the member meets certain criteria. The
$180 reward is applicable to either a participating fitness or gym partner or a non-participating fitness or gym facility. The $180
reward can be issued monthly at $15.00 per month up to twelve (12) months but no more than a maximum reward of $180.00 per
Contract Period. To qualify for the reward, you must be a current NetCare member eighteen (18) years and older and meet a
minimum of eight (8) visits per month to receive the reimbursement for that month. You are required to submit and complete a
reimbursement form and attach copies of your membership contract, proof of payment for your fitness membership, and a monthly
attendance report showing you met the minimum of eight (8) visits per month. For more information, please contact our Customer

5:00pm, Monday through Friday, and on Saturdays from 9:00am – 1:00pm.
