Registration Fees
OSHRM/SOHA members = $100/person
Non-members = $175/person
This fee covers all handout materials, beverage breaks, lunch and administrative costs. Full fee payment must
accompany registration. Send registration and payment to OHA, 155 East Broad Street, Suite 301, Columbus, Ohio
43215. You may register by mail, online or by fax (614-917-2254). Online registration is encouraged for credit card
payments.
Cancellations
A refund of registration fees will be made to those registrants notifying OHA of cancellation three (3) working days prior
to program date. A $40 processing fee will be assessed against each refund. No refunds will be made after that date.
Overnight Accommodations/Parking – Reserve by Feb. 25!
Please contact The Blackwell directly at 614-247-4000 if you need an overnight room. Identify yourself as being with the
OSHRM-SOHA Conference in order to receive the group rate of $135.00/night + tax. The deadline to receive the group
rate is February 25, 2015. Complimentary parking is available to those attendees who use the valet service upon arrival
and pick up a parking voucher at the OHA registration table.

OSHRM/SOHA requests your support again for Reach Out and Read®, which prepares central Ohio’s youngest children to
succeed in school by partnering with doctors to prescribe books and encourage families to read together. Please
consider donating a new book for children under four years of age (no political or religious messages please). Monetary
donations are also appreciated, made payable to Nationwide Children’s Foundation (with Reach Out and Read in memo

Continuing Education Credits
The OHA Research and Educational Foundation (OLN-0017-P) is approved as a provider unit of continuing education by
the Ohio Board of Nursing through the approver unit at the Ohio League for Nursing (OBN-006-92) and provider unit
status is valid through June 2014. Please contact OHA’s Center for Education at 614.221.7614 for information on
continuing education credits.
Services for Physically Challenged Individuals
