2015 PROM DANCE Guest Registration Information - Limit One Guest per OHS Student
If you plan to bring an out of school guest to the Prom Dance, that guest MUST BE REGISTERED by May 1, at 12:00 p.m. in the Activities Office.
There will be NO EXCEPTIONS to this deadline. Overland High School Administration has the right to verify the information provided on the
contract and has the right to refuse admittance of any guest to the dance. No Middle School students and no guest 21 or over will be allowed to
attend the Prom Dance. Cherry Creek School District has a drug-free, alcohol-free, tobacco-free policy. All guests are expected to adhere to this
policy. Your child may use this contract to complete the registration process.

Guest Registration Contract
I agree to supply completely and truthfully the information below. I understand that Overland High School
Administration has the right to verify this information. I also understand that Overland High School has the right to refuse
admittance of my guest to the Homecoming Dance.
The following rules apply:
1. I must return this completed contract by 12:00 p.m. on Friday, May 1, 2015 to the Activities Office with a copy
of my guest’s ID.
2. My guest is under the age of 21, and is not a middle school student. My guest is my responsibility. If he or she is
asked to leave, I will also be asked to leave.
3. All students and their guests, regardless of age, will be held accountable to CCSD rules regarding alcohol, drugs,
and tobacco.
4. I will not be allowed to purchase tickets or be admitted to the dance until this guest registration contract is
completed, signed and returned by the above date.
5. My parent’s signature below indicates their knowledge of my prom dance date and the expectations outlined in
this contract.
6. I will bring a current OHS ID and my guest will bring a current photo ID to the dance.
