Options include having participated in Onsite Biometric Screenings or downloading a personalized Health Provider Screening Form (MD Form) for your in-network physician or Convenience Care Clinic (not an Urgent Care Center) to complete after they do the biometric screenings. Fax your form to Optum as indicated on the form, and follow up to ensure the form was received, if you are not contacted within 30 days. To download your personalized Health Provider Screening Form (MD Form) visit the Health and Wellness Portal on www.myuhc.com . First time users will need to register with information from their United Healthcare ID card. STEP 3 If 2 or more of your biometric measurements do not fall within the established ranges you can still earn rewards by participating in a telephone-based wellness coaching program. To connect one-on-one with a Wellness Coach, call 1-800-478-1057. Programs may take 8 – 12 weeks for completion of the program. The program must be completed prior to the rewards credit due date to be eligible (July 31, 2015) for the reward to begin in January of 2016.
