Instructions to register on the UnitedHealthcare (UHC)
Website and the Health & Wellness Rewards Page
**All SAISD employees and spouses covered under the UHC Medical
plan are eligible to participate and earn up to $175 per person!**

Each employee and covered spouse must register as a NEW user on www.myuhc.com and
must have access to a valid email account for verification to access the UnitedHealthcare
Member website.
Once registered, click on the Health & Wellness Tab  click on Health & Wellness Tools
link and register once more on the Optum Health & Wellness portal.
Please check your email and click on the link provided to validate your email address.
Log back into the Optum Health & Wellness log-in screen provided and this will direct you
to the SimplyEngaged homepage. If you are not redirected, return to the www.myuhc.com
homepage, click on the Health & Wellness Tab  Health & Wellness Tools link to be
redirected to the homepage for the SimplyEngaged Rewards Program.
Click on I AM - click on the orange Health Assessment button to access the questionnaire.
The assessment takes approximately 15-20 minutes to complete. Once completed, you
have earned your first $75 reward.
Click on I DO – to access the online coaching program, choose ONE health coaching topic
and complete the 12 educational activities and 5 weekly trackers listed for that topic. Once
all completed, you have earned your $25 reward.
Call 1-800-478-1057 and select option 3 to connect with a Wellness Coach and complete
3-5 coaching sessions by phone. Phone sessions are approximately 15 minutes in duration
to discuss your personal health goals. Once completed, you have earned your $75 reward.
Click on I GET – click on My Rewards  click on Redeem Rewards link to open the
Rewards page. This page will open in a separate window, if it does not appear, please
“Turn off the Pop-Up Blocker” to open the Rewards page to order your gift cards. E-cards
are electronic gift cards that are sent instantly to your email address and can be redeemed
immediately. Standard plastic gift cards are sent by mail and can take up to 10-20 business
days to be delivered.
All activities must be completed before 10/31/15 to earn your rewards.

provide your basic information for verification.
2. Select the best location, time and date to schedule your health
screening.
3. You will receive confirmation of your scheduled appointment
via email.
*Online registration closes 12 Noon prior to the start of each event.
