SOCIAL ASSISTANCE MAY 2015 CHEQUE ISSUE INFORMATION
Some important dates to remember for May 2015 cheque issue:

ALL RENEWALS & WORK SEARCH FORMS for May 2015 assistance to be turned in by
April 20th 2015.
 MAY 2015 CHEQUE ISSUE- WILL BE FRIDAY
MAY 1ST 2015 at Health Center starting at 10:00 a.m.
Please note that **Late submissions may delay your MAY 2015 cheque.

Also some important reminders:

For Social Developments files to remain in compliance; AANDC requires a bank statement
every quarter. To remain eligible for your May 2015 cheque you will be required to turn in a
bank statement for the entire month of April 2015. *If you do online banking and don’t have
access to a printer you can sign on at the office and print one out. This information remains
confidential and goes into your file. IF you do not have a bank simply write us a note stating
you don’t have a bank account, ensure it is signed and dated.

Social Development will be updating files and will be requiring you to come in to the office for
scheduled appointments- we will contact you for dates and times.

THURSDAYS AND FRIDAYS ARE ADMINISTRATION DAYS ~
NO WALK –INS, NO APPOINTMENTS AND ALL CALLS WILL BE PUT THROUGH TO
VOICEMAIL.
