To accurately complete this form, informing team members of scheduled times for their team, singles and
doubles events and send money for all bowlers listed on this application. THIS INCLUDES SIXTH BOWLER.
A final average sheet from 2013-2014 season along with a current average sheet as of January, 2015 must be
included in this entry form.
Substitution of bowlers must be made either one hour before scheduled bowling time or via email three
weeks before tournament.

PNA NATIONAL BOWLING TOURNAMENT RULES
ELIGIBILITY
Only bona fide members of the POLISH NATIONAL ALLIANCE in good standing as of March 1, 2015
(new members) and must remain in good standing continually to be eligible to participate and collect prizes.
No individual will be permitted to bowl more than once in any event.
HANDICAPS AND AVERAGES
The annual tournament of the POLISH NATIONAL ALLIANCE shall be conducted on a handicap basis.
Each bowler shall receive a handicap 90% of the difference between his/her average and 220 pins in each
event entered. However, under no circumstances shall a man's handicap be over 76 pins per game (135
average) and a ladies' handicap be over 90 pins per game (120 average).
Each bowler shall use the highest previous season's average between September 1, 2013 and May 31, 2014
of any league in which they bowled at least 21 games. The average shall be certified by the league secretary.
If the current season's average is 10 or more pins above previous season's average - the current
season's average (over 21 games) will be used.
If a bowler did not bowl the previous season, his/her current season's average as of January 1, 2015 of at least
21 games must be used. When a contestant has no established average from the previous season, or as of
January 1, 2015, the coordinator shall enter an average of 200 for men, and 175 for women.
If an entrant turns in an average less than required, which results in an additional handicap, the team and
entrant shall be disqualified if such erroneous average is not corrected prior to completion of the first game
of a series.
ANY BOWLER FALSIFYING HIS/HER SCORE, AVERAGE OR IDENTITY WILL BE
DISQUALIFIED FROM ALL EVENTS AND BANNED FROM FUTURE PNA NATIONAL
TOURNAMENTS.
THE AVERAGES WHICH ARE SENT WITH THE TEAMS STANDING SHEET WILL BE USED
AS AN ESTABLISHED AVERAGE. NO CHANGES WILL BE MADE AT THE TOURNAMENT.
WE ALSO ASK FOR YOUR USBC NUMBER AS A SOURCE OF AVERAGE VERIFICATION.
PRIZES
One hundred percent of all prize fees will be returned to the winners. The distribution of the prize
money shall be left up to the PNA National Bowling Tournament Committee.
SUBSTITUTES AND LINE UP CHANGES
No more than two (2) eligible substitutes will be permitted on a team after closing of the entry date - and after
entry has been accepted. Changes should be sent in writing or by email to the coordinator [email protected] NO PHONE CALLS.
There will be no last minute changes in the lineup. The only changes allowed will be in the event of a
substitute bowler, made at least one hour before scheduled time of bowling. All changes MUST be made
first in the tournament office within one hour before the start of any squad.
The PNA National Bowling Tournament Rules and Regulations will govern this tournament and they
will be strictly enforced.
If a bowler arrives late, catching up will not be permitted. If a bowler is not at the alley to bowl in
his/her turn, he/she shall forfeit his/her right to score in said frame. Frames not bowled, must be
counted as "0" (zero). There are no lengthy breaks between games.
Any errors made by manual scorers must be reported by the captain before he/she leaves the bowling
alley. Any errors made by automatic scorers must be brought to the monitor's attention
IMMEDIATELY. All scores are checked with the scores given to us from the bowling alley.
