Re: Prescription Drug Benefit Change
Dear XXXXXXXXXXXXX:
The Arkansas State and Public School Life and Health Insurance Board sets the policy for the health and life insurance
programs for the state and public school employees. The Drug Utilization and Evaluation Committee (DUEC) reviews all
pharmacy benefits and makes recommendations on the prescription drug benefits using evidence-based review. The Board
met and reviewed recommendations from the DUEC in March 2015 and approved the following changes which our records
indicate may affect you.
For existing users, Effective 07/1/15:

The medications Zyflo CR and Zyflo, commonly used to treat asthma, will be non-covered medications. The generic
drug montelukast (generic for Singulair) will remain covered with a Tier 1 copayment but will no longer require prior
authorization.
