Intelligence Testing Requirements
We require all students age 4 and older to submit a psychologist report meeting the following
requirements. Students under 4 will be assessed during scheduled shadow days but will be required
to submit the psychologist report once they turn 4.
When scheduling your appointment for testing, provide the psychologist with the following list of
requirements.
1. Full scale IQ score, subtests score, and all indexes.
2. GAI (if applicable)
3. Child behavioral checklist.
Accepted behavior rating instruments include the BASC and Achenbach. In addition to a behavior rating
scale, the psychologist should provide behavioral observations including clinical impressions of child
behaviors during testing, and the impact the behavior might have on their classroom
functioning performance. If your child is attending school, please include a teacher rating on the
aforementioned scales, as a part of their evaluation.
4. Psychological interpretation of the profile of cognitive abilities as measured by the IQ test.
For example, if a discrepancy is observed during testing, the psychologist should administer additional
testing to explain the discrepancy.
5. If the psychologist notes any areas that need further testing or services, it should be noted in the
report. (e.g. speech and OT.)
