Requirements for top 3 missed standards: Standard 7: The complete education documentation needs to include all the steps indicated in the Education cycle. For sample and templates please see http://professional.diabetes.org/Recognition.aspx?typ=15&cid=93574 Standard 8: The Diabetes Self Management Support (DSMS) is a plan selected by the patient with the assistance of the DSME instructor(s). The plan supports knowledge, skills and behavioral changes gained from the DSME experience. The DSMS plan focuses on accessible, community or web resources available to the patient for ongoing diabetes selfmanagement outside of formal healthcare. Documentation of DSMS must show that it was selected by the patient. Example: Patient chose to join Weight Watchers for DSMS plan. An incorrect example: Patient encouraged joining monthly support group. Standard 9: The program needs to have documentation of selection and followup on at least one behavior goal and one other participant outcome for each patient.
