P.S.11 2015 Yearbook Advertising Form
The P.S.11 Yearbook is distributed to 800 families and staff members who live, work, and shop
in Chelsea. This memorable yearbook is made possible by the generosity of P.S.11 families
and our supporters. Many families choose to congratulate a fifth grade graduating student,
team, or club; acknowledge a teacher; or advertise a small business with an ad in the
yearbook.
To place an ad in the yearbook:
 Complete the Ad Placement form below (choose a full page, half page, or business card
size ad)
 Submit your composed ad as a high-resolution PDF or 300 dpi jpeg on a thumb drive;
please include an actual size, hard copy printout
 Enclose your completed Ad Placement form, payment, thumb drive, and hard copy in a
sealed envelope, and place it in the P.S. 11 main office mail box of our Parent
Coordinator, Stephen McGill
