Tournament Checklist
Registration will be open:
Friday evening from 3:30pm to 7:30pm
Saturday Morning Starting at 7:30am to 1:30pm
All teams MUST be registered (1) One Hour prior to their first game.
To register for the Peoria Soccer Club’s River City Soccer Invitational teams must have COPIES of the following
information at time of check-in.
Current Players Passes Sanctioned by the US Youth Soccer of US Club

Soccer. Recreation passes will not be accepted!

Current State Approved Roster
IYSA Tournament Roster – Please be sure the Player numbers are on

this form.
A copy of the signed and dated IYSA Medical Waivers – All players

must have this form completed including US Club Players.

Guest Player Forms (Maximum 4)

Travel Permit (If your team is from Outside of Illinois)
If you are a US Club Team you must also complete and bring the following material:

NOT BE AVAILABLE. DO NOT MAIL OR EMAIL YOUR REGISTRATION FORMS.
YOUR TEAM WILL BE CHECKED-IN AT THE APPROPIATE FIELD MARSHALL TENT
BEGINNING 45 MINUTES PRIOR TO YOUR SCHEDULED GAME.
