Festival Volunteer Form 2015
June 5, 6 & 7
Please fill out this form and return to school by Wednesday, April 29th. All forms returned will receive an out
of uniform day on Thursday, May 7, 2015. Please fill out the form for the out of uniform even if you have
already volunteered or cannot volunteer.
The summer festival is fast approaching and we need many, many volunteers to make this year a success.
This event is the athletic boards’ major fundraiser and they are asking for your participation. If you have a
child playing a sport(s) at St. Antoninus, please consider committing in some capacity to working the festival.
Obviously, many people commit and work many more hours than that (for which we are grateful), but this is a
suggested commitment. The benefits from this major fundraiser helps keep our pay-to-play fees low and helps
offset various expenses of the club.
How can you help? Please let us know what you are interested in working by checking the spaces below. The
coordinators will be reaching out to you with more information once it gets closer to the festival. We are
appreciative of all those volunteering to make the festival a success. THANK YOU!!!
Fill out and return this form to the office by Wednesday, April 29. Need only one per family.
